SUBJECT: Approval of Contract Amendment Agreement #23-155-4
with Joyce White Associates
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chair, Board of Supervisors, to execute
on behalf of the County, Contract Amendment Agreement #23-155-4,
effective June 30, 1993 , to amend Short Form Service Contract
#23-155-2 (as amended by Contract Amendment Agreement #23-155-3)
with Joyce White Associates, to extend the Contract to August 31,
1993 and to increase the Contract Payment Limit by $5,400, from
$25, 000 to a new total Payment Limit of $30, 400.
II. FINANCIAL IMPACT:
This Contract is included in the Health Services Department's FY
1993-94 budget projection and is funded by the Federal Options
for Recovery Grant. No County funds are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
This Contractor has been providing consultation and technical
assistance, at the request of the Health Services Director, for
the Department's program managers, staff, and participants with
regard to community outreach issues under contract since June 15,
1992 .
Approval of this Contract Amendment Agreement #23-155-4 will
extend the Contract through August 31, 1993 and increase the
number of service units the Contractor will provide.

The Community Services Department has received approval from the
Board for operation of the program at the 2999 Pullman site for the
term of June 1, 1993, through June 30, 1993 . This proposal seeks
to enable the Community Services Director to expedite the process
and enter into the contract and sign documents on this standard
State DOE contract as soon as, received. This will enable the
continuation of those services CSD is already providing for a full
fiscal year.
This Contract will be reviewed and approved by County Counsel as to
form prior to issuance.

AMENDMENT SPECIFICATIONS
Number 23-155-4
In consideration for Contractor's agreement to provide additional
services under the Contract identified herein, County agrees to
extend the term and to increase the Contract Payment Limit. County
and Contractor agree, therefore, to amend said Contract as
specified below, while all other parts of the Contract remain
unchanged and in full force and effect.
1. Extension of Term. The term set forth in Contract
Paragraph 3 . (Term) is hereby extended from June 30, 1993 to
August 31 , 1993
2 . Payment Limit Increase. The Contract Payment Limit set
forth in Contract Paragraph 5. (Payment Limit) is hereby increased
by $5,400, from $25, 000 to a new total Payment Limit of $30,400.
3 . Service Unit Increase. The number of service units set
forth in Contract Paragraph 6. (County's Obligations) , subparagraph
a. is hereby increased by 180 service units from 833 to a new total
of 1, 013 service units.
4 . Modification of Contractor's Obligations. Paragraph 7.
(Contractor's Obligations) of the Contract is hereby modified by
the addition of the following new paragraph:
"During the extended term of this Contract, Contractor
shall provide perinatal outreach services (Cost Center
5826) for 12 hours each calendar week and shall provide
perinatal substance abuse services (Cost Center 5922) for
8 hours each calendar week. "
