SUBJECT: Approval of Medical Specialist Contract #26-881-4 with
Craig Nielsen, M.D.
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee,
(Frank Puglisi, Jr. ) to execute on behalf of the County, Medical
Specialist Contract #26-881-4 with Craig Nielsen, M.D. (specialty:
Anesthesiology) , for the period July 1, 1993 through June 30, 1994, to
be paid as follows:
a. For consultation and training, and/or outpatient clinic
activities, $42 .80 per hour;
b. $27. 00 per RVS Unit for each medical procedure; and
C. For on-call services:
(1) 500 per weekend on-call duty period
(2) 150 per holiday on-call duty period
(3) 50 per weekday evening on-call duty period
(4) 100 per weekday on-call duty period
II. FINANCIAL IMPACT:
Cost to the County depends upon utilization. As appropriate, patients
and/or third party payers will be billed for services.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services which
are not otherwise available in its hospital and clinics.
Approval of Medical Specialist Contract #26-881-4 will allow Craig
Nielsen, M.D. to continue providing Anesthesiology Services for
Merrithew Memorial Hospital and Clinics through June 30, 1994 .

I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee,
(Frank Puglisi, Jr. ) to execute on behalf of the County, Medical
Specialist Contract #26-868-7 with Charles G. Demitz, M.D. (specialty:
Anesthesiology) , for the period July 1, 1993 through June 30, 1994, to
be paid as follows:
a. $42 .80 per hour for consultation and training, and/or
outpatient clinic activities;
b. $27. 00 per RVS Unit for each medical procedure; and
C. For on-call services:
(1) 500 per weekend on-call duty period
(2) 150 per holiday on-call duty period
(3) 50 per weekday evening on-call duty period
(4) 100 per weekday on-call duty period
II. FINANCIAL IMPACT:
Cost to the County depends upon utilization. As appropriate, patients
and/or third party payers will be billed for services.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services which
are not otherwise available in its hospital and clinics.

I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee,
(Frank Puglisi, Jr. ) to execute on behalf of the County, Medical
Specialist Contract #26-817-17 with James M. Rosin, M.D. (Specialty:
Nephrology) , for the period July 1, 1993 through June 30, 1994, to be
paid as follows:
42.80 per hour for the provision of consultation, training
services, and/or medical procedures.
II. FINANCIAL IMPACT:
Cost to the County depends upon utilization. As appropriate, patients
and/or third party payers will be billed for services.
III. REASONS FOR RECOMMENDATIONSL•BACRGROUND:
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services which
are not otherwise available in its hospital and clinics.
Approval of Medical Specialist Contract #26-817-17 will allow James M.

Medical Group (specialty: General, Vascular and Thoracic Surgery) ,

a. For consultation and training, $42.80 per hour, and
b. For surgery, fifty percent (50%) of the fee stated in the
official fee schedule approved by the Division of Industrial
Accidents, State of California, in effect on the date of surgery.
