SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Chair, Board of Supervisors, to execute on behalf of the County, Contract Amendment Agreement #.22-269-12 with Juan Reardon, M.D. , M.P.H. , effective April 1, 1993, to amend Standard Contract #22-269-11 (effective July 1, 1992 through June 30, 1993) , to increase the Contract Payment Limit by $16, 105, from $55, 580 to a new total Payment Limit of $711685 for provision of additional epidemiology consultant and HIV/AIDS surveillance services. d. II. FINANCIAL IMPACT: This contract is funded by the State. No County funding is required. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: This Contractor has been providing consultation, and technical assistance with regard to the AIDS Alternative Test Site Program since 1988. He also provides epidemiology and HIV surveillance services. Approval of Contract Amendment Agreement #22-269-12 will allow the Contractor to provide additional epidemiology consultant and HIV/AIDS surveillance services to the Department' s Assistant Health Services Director of Public Health and the Communicable Disease Control Program Director. County and Contractor agree, therefore, to amend said Contract as specified below, while all other parts of the Contract remain unchanged and in full force and effect. 1. Payment Limit Increase. The Payment Limit specified in Paragraph 4. (Payment Limit) , is hereby increased by $16,105, from $55.580 to a new total payment limit of $71,685. 2. Increased Service Units. The number of service units specified in Payment Provisions Paragraph 1. (Payment Amounts) subparagraph d. is hereby increased by 473.68 service units, from 2080 to a new total of 2555.68 service units. 3. Increased Hourly Rate. The hourly rate specified in Payment Provisions Paragraph 1. (Payment Amount) , subparagraph d. is hereby increased from $26 .00 per hour to a new hourly rate of $34.00 per hour. 4. Service Plan Modification. Service Plan Paragraph 1. (Service Specifications) is hereby amended by the addition of the following subparagraphs: "c. Contractor shall provide consultation and technical assistance to County's AIDS Program Director, as required, regarding: (1) Surveillance of AIDS cases and seroprevalence in Contra Costa County; (2) Epidemiology services including directing the activities of other individuals providing surveillance and epidemiology services; (3) Research design, implementation and evaluation of HIV serosurveillance and epidemiologic efforts; and (4) Analysis of epidemiology of AIDS and HIV infection and the preparation of documents and materials for presentation of this information to Health Services Department's Senior Staff, other policy makers and the public. d. Contractor shall provide consultation and technical assistance to the Assistant Health Services Director for Public Health and Communicable Disease Program Director regarding: (1) Potential or active epidemic trends, locations and sources, by monitoring case reports, health statistics, demographic information.and results of laboratory tests; (2) Design of epidemiological surveillance and investigative programs including defining problems areas, performing and supervising the collections, analysis interpretation and evaluation of data; and (3) Status of public health in Contra Costa County as compared to national objectives. e. Contractor shall serve as a resource and a technical consultant on public health epidemiology issues to physicians, nurses, public health officials and the public.
