SUBJECT: Approval of Contract Amendment Agreement #24-460-11 � ��``11
with Phoenix Programs, Inc.
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chair, Board of Supervisors, to execute on
behalf of the County, Contract Amendment Agreement #24-460-11,
effective February 1, 1993, to amend Novation Contract #24-460-9 (as
amended by Contract Amendment Agreement #24-460-10) , with Phoenix
Programs, Inc. , to modify the Contract Payment Provisions.
II. FINANCIAL IMPACT:
None. This Contract Amendment Agreement modifies the Payment
Provisions of Contract by reducing the fee rate County will pay for
Nevin House Transitional Residential Services to accurately reflect
the Contractor's costs for the services. There is no change in the
Contract Payment Limit. The Contract is funded 100% by Standard
Agreement #92-72122 with the State Department of Mental Health (County
Contract #29-441-9) , which finances the County's Conditional Release
Program (CONREP) for mentally disordered offenders.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On December 15, 1992, the Board of Supervisors approved Novation
Contract #24-460-9 with Phoenix Programs, Inc. for services to CONREP
clients. Contract Amendment Agreement #24-460-10, which was approved
by the Board on January 26, 1993, amended the Contract to replace the
Nyumba Chuki facility which provided transitional residential care
with Nevin House.
The purpose of Contract Amendment Agreement #24-460-10 is to amend the
Contract to adjust the payment rate to accurately reflect the cost of
the service which the Contractor provides at Nevin House under the
terms of this Contract.

AMENDMENT SPECIFICATIONS

In consideration for Contractor's willingness to provide its Nevin
House Transitional Residential Services at a fee rate adjusted to
cost under the Contract identified herein, County agrees to amend
the Contract Payment Provisions. County and Contractor agree
therefore to amend said Contract as set forth below while all other
parts of the Contract remain unchanged and in full force and
effect.
1. Modification of Contract Payment Provisions. Contract
Payment Provisions Paragraph 1. (Payment Amounts) , subparagraph d.
is hereby amended by the addition of a new sub-subparagraph (7) to
read as follows:
11 (7) $ 80 .53 for each Nevin House Residential Service Client-
