Approval of Contract Amendment Agreement #22-378-3 with
East Bay Perinatal Council
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chair, Board of Supervisors, to
execute on behalf of the County, Contract Amendment Agreement
#22-378-3 with East Bay Perinatal Council effective March 1,
1993 , to amend Standard Contract #22-378 (as amended by
Contract Amendments #22-378-1 and #22-378-2) effective January
1, 1991 through June 30, 1993) , to increase the payment limit
by $28, 162 , from $3251726 to a new Contract payment limit of
$353,888. This Contractor provides case management services
to African American high risk pregnant women and/or high risk
women who have recently delivered high risk infants in West
Contra Costa County.
II. FINANCIAL IMPACT:
This Contract is funded 100% by a State grant for the
Department's West County African-American Infant Health
Project. No County match is required.
III. REASONS FOR RECOMMENDATIONS/.BACKGROUND:
On September 17, 1991, the Board approved Standard Contract
#22-378 (as amended by Contract Amendment Agreements #22-278-1
and #22-278-2) with the East Bay Perinatal Council to provide
case management, education, and outreach services to African
American pregnant or parenting women and. their infants in the
Richmond area.
Approval of Contract Amendment Agreement #22-378-3 will allow
the Contractor to provide additional outreach services and
support group activities for clients enrolled in the Child
Health and Disability Prenatal Care Guidance Program.

/4 AMENDMENT SPECIFICATIONS

In consideration of the Contractor's willingness to expand services to the Department's
African American High-Risk Infant Project, as set forth in the Contract identified herein (as
amended by Contract Amendment Agreements #22-378-1 and #22-378-2) , County and Contractor
agree to amend said Contract as specified below while all other parts of the Contract remain
unchanged and in full force and effect.
1. Payment Limit Increase. The Payment Limit set forth in Paragraph 4. (Payment Limit) is
hereby increased from $325,726 to a new total Contract Payment Limit of $353,888.
2. Payment Provisions. Payment Provisions Paragraph 1. (Payment Amounts) subparagraph d. ,
is hereby modified to read as follows:
" [X] d. (1) $40.706.00 payable upon execution of this Contract; and
(2) After approval by the Department of properly documented D-15 demand
forms, and of Contractor's quarterly reports set forth in Service Plan Paragraph 5. (Progress
Reports) :
(a) $20.353.50 payable quarterly, on or after the first day of each
of the following months: October 1991 and January 1992; and
(b) $40,718.00 payable quarterly on or after the first day of each
of the following months: April, 1992; July 1992; October 1992 and
January 1993; and
(c) $47.758.00 payable on or after April 1, 1993; and
(d) $61.845.00 payable on or after July 1, 1993. "
3. Service Specificiations. Service Plan Paragraph 3. (Service Specifications) is hereby
modified as specified below:
a. Subparagraph b. (Case Management Services) , is hereby modified to add the following
services:
"(12) Contractor shall expand existing support groups by one, to focus on male
partners of clients enrolled in the CHDP - PCGP.
(13) Contractor shall expand outreach efforts to include flyer distribution and other
visual aides aimed at informing more pregnant women about the need for early entry into
ongoing prenatal care. "
b. Subparagraph c. (Project Coordinator) , is :hereby modified to add the following
services:
"(7) Analyzing program data to determine the extent to which existing goals and
objectives are being met.
(8) Collecting materials and data, providing a written report, and making
recommendations to the Health Services Director (or his designee) with regard to future needs
and services.
(9) Developing a strategic plan to continue advocacy services for Black Infant Health
Project once current funding cycle has ended, including identification of other possible
funding sources, grant writing, collaboration efforts and integration with other agencies and
organizations with a focus on providing services to High Risk African American pregnant women
and their infants." InA
