PC-9293-01 SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION Authorize Director, Office on Aging, or his designee, to submit the Project Care Continuing Application for Second Year,. County # 29-502-1 from the California Department of Aging to the Contra Costa County Office on Aging effective August 1, 1992 - July 31, 1993 in the amount of $30, 000, to continue a local coalition building effort and to implement services to frail elders in Richmond and San Pablo based on the needs assessment of the Project Care Health Services Task Force. In addition, authorize the Director, Office on Aging, or his designee, to sign the State Contract, County # 29-502-1 upon approval of the Project Care Continuing Application for Second Year by the California Department of Aging. II. FINANCIAL IMPACT The Project Care Continuing Application for Second Year, County # 29-502-1, has a total budget of $30, 000, of which $20, 000 will be subcontracted to assist frail elders in Richmond and San Pablo. The Office on Aging retains $10, 000 for services it performs. The funding is 100% federal dollars and there is nc cost to the county. BACKGROUND The Contra Costa County Office on Aging is applying for second year funding for the Project Care Grant, County # 29-502-1 from the California Department of Aging. During the initial grant period, the Office on Aging was selected to administer the grant and subcontracted with the Richmond Chamber of Commerce to do a needs assessment.
