MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: June 4, 1991 SUBJECT: Approve Contract Change Order No. 6 for the Appian Way Widening Project Project No. 0662-6134288-84 Specific uest s or Recommendation(s)&Bac grow n t. Recommended Action: APPROVE Contract Change Order No. 6 in the approximate amount of$900.00 and AUTHORIZE the Public Works Director to execute it in order to compensate the contractor, Gallagher & Burk, Inc., to place slurried concrete around a RCP storm line. II. Financial Impact: The project contingency funds are sufficient to cover this change order cost. It is funded by Federal Highway Administration Funds, (FAU 86%) and Local Road Funds (14%). III. Reasons for Recommendations and Background: The location of this pipe required added protection and strength. Also this change order plus change orders 1 thru 5 exceed $25,000.00 thereby requiring Board approval. IV. Consequences of Negative Action: Lack of APPROVAL would preclude payment to the contractor for work required.
