TO PROVIDE IN-HOME SERVICES REGISTRY AND HOME VISITING FOR SENIORS IN CONTRA COSTA COUNTY SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION Approve the recommendation of the Director, Office on Aging to award contracts to the following agency in response to Request for Proposal (R.F.P. ) #004 for In-Home Services Registry and RFP #005 for Home Visiting: Contractor Services Term Amount Family and Home Visiting 7-1-91 $21, 047 Community Services, 6-30-92 of Contra Costa, Inc. Family and In-Home 7-1-91 $33, 066 Community Services 6-30-92 Services Registry of Contra Costa, Inc. In addition, authorize the Director, Office on Aging or his designee, to conduct contract negotiations. Also, authorize the Director, Office on Aging to sign the contracts, upon approval by County Counsel or the Social Service Department' s Contracts & Grants Administrator, with Family and Community Services of Contra Costa, Inc. for the service, term, and amount specified above. Contracted funds are all federal and state dollars. III. REASONS FOR RECOMMENDATION/BACKGROUND The California Department of Aging requires the Office on Aging to go to bid for their contracted out services at least every four (4) years. In March, 1991 the Office on Aging began the R.F.P. process for Home Visiting and In-Home Services Registry to be funded through Older Americans Act Federal and State funds as allocated in the Area Agency on Aging Four Year Plan. The Office on Aging received one (1) proposal, for Home Visiting and and one (1) for In-Home Services Registry. The proposals were rated and ranked by an evaluation team. The Executive Committee of the Advisory Council on Aging recommends Family & Community Services of Contra Costa, Inc. to provide Home Visiting and In-Home Services Registry programs as specified above.
