MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: May 14, 1991 SUBJECT: APPROVE Contract Change Order No. 5 for the Appian Way Widening project. Project No. 0662-6R4288-84 specific Requests or Recommendation(s) &Background&Justffication I. Recommended Action: APPROVE Contract Change Order No. 5 in the approximate amount of $1,100.00 and AUTHORIZE the Public Works Director to execute it in order to compensate the contractor, Gallagher & Burk, Inc., for an increased cost to re-adjust manholes. II. Financial Impact: The project contingency funds are sufficient to cover this change order cost. It is funded by Federal Highway Administration funds, (FAU-86%) and Local Road Funds (14%). III. Reasons for Recommendations and Background: Manhole structures were located in an area of the driveways making them unsafe for auto traffic. Also this change order plus change orders 1,2,3 and 4 exceed $25,000.00, thereby requiring Board approval. IV. Consequences of Negative Action: The lack of APPROVAL would preclude payment to the contractor for work required by the Construction Division.
