PRENATAL GUIDANCE PROGRAM
Public Health Nurse
Job Description
Women enrolled in Healthy Start are not eligible for these services
unless they miss a minimum of two appointments .
Tasks
1 . Provides on-going overall case management for all pregnant
women identified.
2 . Contacts all women referred within 5 working days of receipt
of referral by phone or home visit based on psycho-
social/medical needs .
3 . Assist the woman in overcoming barriers which prevent making
and keeping appointments with medical providers and Social
Services .
4 . Educates pregnant woman on value and components of prenatal
care and value of preventive health care and immunizations for
her infant .
5 . Educates client on the signs of premature labor and what to
do if they occur.
6 . Clarifies for client her responsibilities to provider (keeping
appointments or calling ahead if unable to keep appointment,
etc. ) .
7 . Assists in identification of special health needs and
referral , and assists to ensure compliance with special health
follow-up.
8 . Assists in identifying appropriate provider for prenatal
services which meets the individual needs of the client, such
as client' s risk status .
9 . Refers pregnant women and pregnant teens to appropriate
community resources as needed by the individual (WIC,
substance abuse center, housing, etc . ) .
10 . Magi keep- statistical data necessary for program development/
evaluation.
11 . Maintains nursing record within Health Department standards .
