Authorize distribution of the Request for Proposal (RFP) for Emergency Ambulance Service; 2. Direct the Health Service Director to report back to the Board on the recommendation for emergency ambulance service providers for Emergency Response Areas 1, 2, 4 , and .5; 3 . Authorize the Health Services Director to negotiate an emergency ambulance service agreement with Moraga Fire Protection District for Emergency Response Area 3; and 4 . Authorize Health Services Director to extend the existing emergency ambulance service agreements through January 2, 1991 . FISCAL IMPACT: No General Fund impact. It is anticipated that, as a result of this selection process, the County will enter into agreements which identify a subsidy for increased levels of emergency ambulance service. Funds for this subsidy will be available through the benefit assessment for County Service Area EM-1 (Measure "H" ) . DEPUTY BACKGROUND: On November 28, 1989 , the Board of Supervisors adopted Resolution No. 89/742 establishing CSA EM-1 to provide enhanced emergency medical services countywide. This followed the passage of Measure "H" in November 1988 calling for a property assessment to fund increased paramedic services and other improvements in the Countywide EMS system and the subsequent agreement of each of the 18 cities for inclusion of their respective territories within a countywide CSA for emergency medical services . The attached RFP solicits proposals for emergency paramedic ambulance service for four of the County' s five emergency response areas : ERA 1 - All of west County; and the central County cities of Martinez, Pleasant Hill, Lafayette, Orinda, Walnut Creek west of I-680 , and surrounding unincorporated areas . (Currently served by Regional Ambulance. ) ERA 2 - Central County cities of Concord, Clayton, Walnut Creek east of I-680, and adjacent unincorporated areas . (Currently .served by Regional Ambulance. ) ERA 4 - Danville, San Ramon, and unincorporated areas of the San Ramon Valley. (Currently served by San Ramon Valley Fire. ) ERA 5 - All of east County. (Currently served by Regional Ambulance . ) Moraga Fire Protection District, the paramedic provider for ERA 3 , is exempted by statute (Health & Safety Code Section 1797 . 201 ) from the competitive selection process and intends to continue providing paramedic ambulance service within ERA 3 at a level with meets or exceeds countywide standards . Some of the increased performance standards set by the RFP include: - paramedic level ambulance response to all medical emergencies (except multiple unit response) within 10 minutes for urban areas or 20 minutes for rural areas (response time standard to be met for 95 percent of calls ) ; - three-person staffing of ambulances during long transports (over 15 minutes ) or critical patients to the trauma center; and - establishment of a physician or nurse directed quality assurance program. In order to assess the feasibility of further enhancing our response time standard, proposals are required to include a deployment plan and associated subsidy requirement for both a 10- minute and a 8-minute response time standard. The selection process will include appointment of a Proposal Review Committee to make recommendations to the Health Services Director regarding ambulance provider selection. ' The Proposal Review Committee will include members of the Measure "H" Steering Committee and other knowledgeable individuals, including persons from outside Contra Costa County. Recommendations will be presented to the Board for authorization to negotiate emergency ambulance contracts . It is anticipated that these agreements will be for a term of two years with an option to renew for an additional two year period. The RFP contains a provision for a formal annual performance evaluation to be conducted by a committee including two representatives each from the Health Services Department, the contract provider, and the Emergency Medical Care Committee. The evaluation committee will report its finding to the Health Services Director and to the Board within 90 days following the end of each contract year.. Existing emergency ambulance service agreements expire June 30, 1990 . In order to provide adequate time for preparation and review of proposals, selection of contractors, and startup of operations, a six month extension of these agreements through January 2, 1991, is being recommended. cc: Moraga Fire Protection District San Ramon Valley Fire Protection District Regional Ambulance
