SUBJECT: Approval of Contract Extension Agreement #24-525-1 with
Lewin/ICF
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Stuart McCullough) to execute on behalf of the County, Contract
Extension Agreement #24-525-1, to extend Standard Contract #24-525
(effective July 11, 1989 through June 30, 1990) with Lewin/ICF through
August 30, 1990 with no change in the Contract Payment Limit.
II. FINANCIAL IMPACT:
None. Approval of this Contract Extension Agreement provides for an
extension of time only with no increase in the Contract Payment Limit.
The Contract is funded by the State Department of Mental Health under
Standard Agreement #29-457 .
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On July 25, 1989, the Board approved Contract #24-525 with Lewin/ICF
for evaluation of interagency collaborative planning and services for
infants and preschoolers as required by Standard Agreement #29-457 with
the State Department of Mental Health.
Approval of Contract Extension Agreement #24-525-.1 will extend the
Contract for sixty days to allow the Contractor additional time in
which to complete the work set forth in the Contract.
