MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: May 2, 1989 SUBJECT: APPROVE Contract Change Order No. 3 for the Cummings Skyway Slide Repair. Project No. 0671-6R6670-86, in the Crockett area. Specific Request(s) or Recommendation(s) & Background & Justification RECOMMENDED ACTION: APPROVE Contract Change Order No. 3 and AUTHORIZE the Public Works Director to execute Contract Change Order No. 3 in the approximate amount of $2 , 300. 00 to compensate the contractor, Holman D. Pettibone, Inc. for increases in contract quantities. FINANCIAL IMPACT: The project is 100% funded by the Federal Highway Administration Emergency Repair fund. REASONS FOR RECOMMENDATION/BACKGROUND: During construction the need for fencing became apparent. Fencing was required for safety and liability reduction purposes. In addition the amount of compensation for items that overran required adjustment. This contract change order provides for the additional work. The aggregate amount of change orders exceeds $25, 000. 00, requiring Board approval. CONSEQUENCES OF NEGATIVE ACTION: The lack of approval would preclude payment for work required to complete theā€žrepair of the slide.
