HomeMy WebLinkAboutMINUTES - 05111988 - 1.54 TO BOARD OF SUPERVISORS FROM: Mark Finucane, Health Services Director Cwtra Costa DATE: April 27, 1988 Cour t/ SUBJECT: Application for Grant to Develop a Community Oriented ��ll��// Primary Care (COPC ) Demonstration Program in Contra Costa SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION RECOMMENDED ACTION : Ratify the actions of the Health Services Director on March 29, 1988 in submitting an application , in conjunction with Dr. Joyce Lashof, Dean of the U . C. Berkeley School of Public Health, to the Kellogg Foundation for approximately 1 .6 million dollars to develop a Community Oriented Primary Care demonstration project in Contra Costa County for a four year period. Authorize the Health Services Director to function as a Principal Investigator for this project . FISCAL IMPACT: The grant proposal will be for approximately 1 . 6 million dollars or $400, 000 per year for four years . The funds would be managed by the Western Consortium for Health Professionals , a nonprofit agency. Mark Finucane and Joyce Lashof as Principal Investigators -would oversee the expen- ditures of funds in accordance with the contract with the Kellogg Foundation , so that this money would be spent to directly promote integrated preventive and primary health care services in Contra Costa County. REASONS FOR RECOMMENDATION: Community Oriented Primary Care, a concept developed by the Institute of Medicine, tailors primary health care delivery centers (clinics , hospitals ) to the health needs of the com- munities served . COPC links public and preventive health with primary care and involves community agencies and orga- nizations in coordinated efforts to promote community health and well being . As an integrated agency which combines Public - Health with a hospital , an extensive clinic system, and a prevention oriented HMO, the Health Services Department has promoted the COPC concept . The Department has also promoted links with community agencies and organi - zations through the advisory boards , the AIDS Task Force, and the work 'of the Prevention Program. Berkeley School of Public Health to implement a pilot COPC project in Richmond, to promote access to perinatal care . COPC started approximately 25 years ago with the establish- ment of federally funded neighborhood health centers to pro- vide medical care, community organizing , and health promotion programs to nearby residents . The Kellogg Foundation is interested in expanding the model and demonstrating the applicability of COPC to a variety of set- tings across the country. The proposed COPC project in Contra Costa County will be the first time this model has been tried in an entire county in conjunction with the local health services department . Contra Costa County, with its diversity . of rural , suburban , and urban areas . and its mix- ture of industrial , farming , and office/service industries , along with a ethnic distribution that reflects the national average, provides an opportunity to demonstrate the applica- bility of COPC in a variety of settings . The primary participants in this program will be the Health Services Department , the University of California School of Public Health , and at least two dozen community agencies or organizations and primary care providers in Contra Costa . Contra Costa ' s rich network of effective community agencies and primary providers , and the Health Services Department ' s history of cooperative and supportive relations with com- munity agencies , provide the setting for a successful demonstration of COPC. -2-
