March 10, 1988 County SUBJECT: Authorization for Expenditure of Funds Under the I County AIDS Case Management Project SPECIFIC REQUEST(S) OR RECOMMENDATIONS) & BACKGROUND AND JUSTIFICATION I . RECOMMENDED ACTION : Authorize the Auditor-Controller to pay, effective March 1 , 1988 , certain demands submitted , and funded , under the AIDS Program Case Management Project to provide food supplementation, housing subsidies , and transportation for people with AIDS who are without other sources of funds , as follows : 1 . Food Vouchers . The AIDS Case Manager , or his/her .designee , will purchase , through the County ' s Purchase Order process , food vouchers which are redeemable at County supermarkets up to a maximum expenditure of $6 , 000. The monthly maximum per person for said vouchers shall be $200, and vouchers will specifiy that they are redeemable for food items only, excluding alcohol and tobacco ; 2 . Subsidized Housing . Upon recommendation of the AIDS Case Manager , or his/her designee , housing costs may be sub- sidized up to $500 per person per month , not to exceed a maximum expenditure of $9 , 000. Payment shall be made upon submission by a landlord or mortgage company, and approval by the Department , of a County Demand Form D-15; and 3. Transportation . Transportation costs up to a maximum expenditure of $2 , 000 may be approved by the AIDS Case Manager , or his/her "designee , for non-emergency transpor- tation for AIDS victims , in accordance with the procedure for non-emergency patient transportation, established by the Health Services Department. II . FINANCIAL IMPACT: Under Standard Agreement #86-89794 approved by the Board on September 15 , 1987 , the County received $100 , 000 in funding from the State Department of Health Services for the AIDS Case Management and Attendant Care Project . No additional County contribution is anticipated . III . REASONS FOR RECOMMENDATIONSIBACKGROUND: On September 15 , 1987 the Board approved Contract 429--353-,1 ( Standard Agreement #86-•89791 ) with the State Department of Health Services for the period June 1 , 1987 through June 30 , 1988 , to implement an AIDS Case Management and Attendant Care Project which is operated within Public Health Division' s AIDS Program. This funding enabled the Department to establish centralized case management for AIDS and ARC patients , a data collection and service tracking system to study cost of care , and home attendant care services . The State recently advised the Department verbally that the Contract is being amended to increase the scope of services pro- vided under the case management component to include assistance to clients with regard to necessities of life such as food , housing , and transportation , and to increase funding for these additional services . The State has sent the Department a draft amendment agreement , to be effective January 1 , 1988 , and expects to complete the final amendment within a few weeks . The AIDS Program has implemented many of the services described in the Contract Scope of Work, including the selection of a Case Manager , and the original funding has not yet been depleted . Since there are clients with immediate need for the additional services described in the State ' s draft amendment , the Department is requesting that the Auditor-Controller be authorized to disburse funds as described in Section I . (Recommended Action) of this Board Order , pending approval by the Board of the State ' s contract amendment . I
