Spooner , Contracts Administrato Costa DATE: April 22, 1987 ^, fty SUBJECT: Approve Submission of Funding Application #29-208-31 to the State Department of Health Services for continuation. of the Immunization Assistance Program SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I . RECOMMENDED ACTION: Approve and authorize submission of Funding Application #29-208-31 to the State Department of Health Services in the amount of $38 , 792 for the period July 1, 1987 - June 30, 1988 for continuation of the Immunization Assistance Program. II . FINANCIAL IMPACT : Approval , of this agreement by the State will result in $38, 792 of State funding , for the Immunization Assistance Program. Sources of funding are as follows : $38, 792 State Funding 18, 944 County Personnel Costs 5,040 County In-Kind (Fixed Overhead) $62 , 776 Total Program The County received $37 , 760 of funding from the State for this program last fiscal year . III . REASONS FOR RECOMMENDATIONS/BACKGROUND: On August 19 , 1986 the Board approved Contract 429-208-30 with the State Department of Health Services for continuation of the long-standing Immunization Assistance Program operated by the Public Health Division of the Health Services Department . Funding ,Application #29-208-31 requests State funding to con- tinue services during FY 1987-88 . Contra Costa County maintains this program to make immunizations available to all persons in need of this service , assisting all county school districts , private schools , preschools and day care centers in meeting the State-mandated immunization requirements . The program goals are to prevent the occurrence and transmission of childhood diseases preventable through immunization and to maintain immunization levels at 98% or greater for school age children and 95% or greater for infants and preschool children. This application must be approved in order for the County to continue to . receive free vaccine from the State . In order,: to meet the State ' s deadline for submission, copies of the application have been submitted to the State , but subject to Board approval . After Board approval , six certified copies of the Board Order should be returned to the Contracts and Grants Unit for 'submission to the State Department of Health S rvices . DG:gm ..
