Spooner , Contracts Administrato ra Costa"J DATE: April 16, 1987 COJ Approval of Standard Agreement (Amendment) #29-315- with the SUBJECT: State Department of Health Services (State #86-88668 Al) for Preventive Health Care for the Aging Program SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I . RECOMMENDED ACTION: Approve and authorize the Chair to execute on behalf of the County, Standard Agreement (Amendment ) 429-315-5 (State 486-88668 Al) effective January 1, 1987 with the State Department of Health Services to amend Standard Agreement 029-315-3 (effective July 11 1986 - June 30, 1987 ) to purchase equipment for the Preventive Health Care for the Aging Program with a $3 , 225 increase in the contract payment limit . II . FINANCIAL IMPACT: Approval of this amendment by the State will result in increased State funding of $3, 225 , from $45 , 000 to a new total payment limit of $48 , 225 . There is no additional County cost under this amendment . III . REASONS FOR RECOMMENDATIONS/BACKGROUND : On October 71 1986 the Board approved Contract #29-315-3 with the State Department of Health Services for continuation of the Preventive Health Care for the Aging Program, also known as the Healthy Older Adult Program (HOAP) . This program provides pre- ventive health education and health promotion activities to the elderly in order to help senior citizens maintain or improve their health through health problem identification , referral for problems found , health counseling and education. Amendment #29-315-5 increases the State ' s funding to allow purchase of equipment for this program. This document has bVen approved by the Department ' s Contracts and Grants Administrator in accordance with the guidelines approved by the Board ' s Order of December 1 , 1981 (Guidelines for contract preparation and processing , Health Services Department) . The Board Chair should sign eight copies of the contract , seven of which should then be returned to the Contracts and Grants Unit for submission to State Department of Health Services .
