Psychiatric Health Facility
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION
Accept report and approve recommendation of Health Services
Director with respect to the proposal of National Psychiatric
Centers, Inc. , to establish a psychiatric health facility in the
Pleasant HIll area.

BACKGROUND
The Board on January 27 , 1987 referred to the Health Services
Director the proposal of National Psychiatric Centers, Inc. ,
(NPC) to open a 40-bed adult psychiatric health facility at 40
Boyd Road in Pleasant Hill. This facility, known as the Pleasant
Hill Retirement Home, is currently operated as a 40-bed geriatric
residential care facility. The proposed facility will be a
voluntary unit, although there may be one restraint and seclusion
room. NPC has invited a number of local psychiatrists to form a
limited partnership in developing this project. A majority of
the patients will come from Contra Costa County, although
referrals will be accepted from out of County. The anticipated
average length of stay is 10-14 days. NPC estimates that a high
percentage of its operating revenue will be private insurance.
The cost per day will be about $350, exclusive of provider costs.
A psychiatric health facility (PHF) can only operate at a daily
rate that is 60 percent of the area average of acute psychiatric
hospitals. NPC expects the facility to be JCAH and Medicare
approved.
NPC is not interested in contracting with Contra Costa County for
indigent patients. NPC will forward to the Health Services
Department copies of its interfacility transfer agreements, which
are referenced in the proposal. NPC does not intend to apply for
5150 designation. NPC has recently concluded a lease-purchase
option on the property contiguous to the proposed site should it
decide to expand in the future.

Staff from the National Psychiatric Centers have been in contact
with our patients ' rights organizations. We have written to
National Psychiatric Centers requesting their consideration of
assuming part of the costs that will be required in providing
patients ' rights advocacy.
A major concern will be the placement of the existing geriatric
population located at the residential care facility. There may
be opposition from geriatric advocates who are concerned about
where the current occupants will be relocated. There is
particular concern regarding those people who rely on SSI as
their main source of income.
The State Department of Mental Health indicates the program will
probably "fall within the range of approvability" , although they
are interested in our recommendations. Therefore, we recommend a
letter be sent by the Mental Health Director to the State
Department of Mental Health recommending the following.
(1) Approval of this application will be subject to assuring
comparable and appropriate placement for the residents of
the residential care facility. This will include placement
for those individuals who rely on Supplemental Security
Income. The A/DA/MH Division Geriatrics Unit will monitor
and report to the Director regarding these placement
proceedings.
( 2) National Psychiatric Centers will enter into negotiations
with the Health Services Department to defray part of the
cost of providing patients ' rights services at the facility.
These recommendations have been reviewed by the Mental Health
Advisory Board.
