February 20, 1987 Courcy SUBJECT: RELIEF OF CASH SHORTAGES - PUBLIC HEALTH DIVISION 1 „1 SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION RECOMMENDATION• Authorize relief of a cash shortage of $50 in the Public Health Division, as recommended by the District Attorney and the Auditor-Controller. BACKGROUND: The Health Services Public Health Division sustained three individual cash shortages from its petty cash revolving fund totalling $50 . The shortages . were sustained by the Richmond Public Health Office (Budget Unit 0450) and occurred during the period of August 9, 1984 through December 12, 1984. Investigation by the Public Health fiscal unit disclosed two reported thefts totalling $20 and a previously undetected loss of $30 which occurred between 10-1-84 and 12-10-84 at the Richmond Public Health Office. Several changes in personnel occurred during that period which reduced the division' s ability to resolve the unidentified shortage. At the time each shortage was discovered, efforts were made by the staff to ascertain and recover the shortage amount. These efforts were not successful at that time and further efforts could not be cost justified. Because building security seemed to be beyond the division' s control, arrangements were made to transfer all Public Health Nursing change funds to the general Petty Cash Fund maintained by the Martinez Administrative Office. Only the daily counter receipts are present at any given time. . The existing control system ' for those receipts should preclude the recurrence of any future thefts. The matter was investigated by the District Attorney and the Auditor-Controller. Having found no evidence of either fraud or negligence, it is recommended that the Public Health Division be relieved from the shortage and the amount be deemed a charge against the General Fund. BACKGROUND: On June 23, 1986, the amount of $469 . 69 in cash was discovered to be missing from the Loomis deposit box kept in the walk-in safe in the patient registration area of the Hospital. The security officer on duty notified the Martinez Police and a report was made. The matter was investigated by the District Attorney and the Auditor-Controller. Having found no evidence of either fraud or negligence, it is recommended that the Health Services Department be relieved from the shortage and the amount be deemed a charge against the General Fund.
