Spooner, Contracts Administrator Costa DATE: July 3, 1986 lMjqy SUBJECT: Approval of Contract Amendment Agreement 422-182-4 with Tri Valley Rehabilitation SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Chairman to execute on behalf of the County, Contract Amendment Agreement X622-182-4 with Tri Valley Rehabilitation effective May 20, 1986 to amend Contract 422-182-1- (effective January 1, 1985) for provision of additional pediatric therapy services with a $30,000 increase in the contract payment limit. II. FINANCIAL IMPACT: Approval of this amendment will result in an increase in the contract payment limit of $30,000, from $76,320 to a new total payment limit of $106,320. Actual cost to the County depends upon utilization. There is no increase in the $26.50 hourly rate. Sources of funds for this contract are the State (80%) and the County (20%), and funding is included in the FY 1985-86 Department budget. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: The Purchasing Agent approved Contract 422-182-1 with Tri Valley Rehabilitation effective January 1, 1985 through December 31, 1985. On November 5, 1985, the Board approved Contract Amendment Agreement 422-182-2 to extend services through June 30, 1986, increase the contract payment limit and number of service units pro- vided, and add the services of a pediatric occupational therapist. On May 11, 1986, the Board approved Contract Amendment Agreement #22-182-3 to increase the contract payment limit and units of services provided by contractor. Despite extensive advertising, the Department has been unable to fill two therapy posi- tions . The contractor has been used to fill in as required to relieve or augment the Department's therapy staff. Until April 1, 1986, Tri Valley Rehabilitation could only supply 1.4 full-time equivalent (FTE) therapists even though the Department had two vacant positions. For the months of April, May and June, the contractor was able to provide 3.7 full-time equivalent therapists . By use of these additional services, the County has been able to expedite treatment for clients for whom services had been delayed due to vacant County positions and una- vailable contract therapists. This document has been approved by the Department's Contracts and Grants Administrator in accordance with the guidelines approved by the Board's Order of December 1, 1981 (Guidelines for contract preparation and processing, Health Services Department) .
