SUBJECT: Approval of Standard Agreement for Local Jurisdiction Special Needs and
Priorities Funding (SNAP) 429-639 with the State Department of Health Services
(Distressed County Facilities State Project #VI-106)
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) &' BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chairman to execute on behalf of the County, Standard
Agreement/SNAP 429-639 (State Project #VI-106) with the State Department of Health
Services in the amount of $85,000 for the period May 31, 1986 - May 31, 1987 for
renovation and repairs at Merrithew Memorial Hospital.
II . FINANCIAL IMPACT:
Approval of this agreement by the State will result in $85,000 of State funding for
renovation and repairs at Merrithew Memorial Hospital. This project is 100% State
funded with no County match required.
III . REASONS FOR RECOMMENDATIONS/BACKGROUND:
Each year the State makes AB-8 Special Needs and Priorities Funds (SNAP) available
to counties . 'On March 18, 1986, the Board authorized submission of applications
for various projects under the Distressed County Facility, Refugee Health Services,
Computerized Information Systems, and Elderly Care categories for Special Needs and
Priorities funding. Standard Agreement #29-639 will provide $85,000 of State
Distressed Facilities SNAP funds to remodel and replace portions, of the roof,
install one and two-hour fire separation walls, and install a new pad and replace
the wall around the oxygen tank at Merrithew Memorial Hospital. These improvements
are required to assure a continued . safe and healthy environment for patients and
staff.
Renovation and repairs will be contracted through the Public Works Department with
appropriate bidding processes for local contractors to do the work.
This document has been approved by the Department's Contracts and Grants
Administrator in accordance with the guidelines approved by the Board's Order of
December 1, 1981 (Guidelines for contract preparation and processing, Health
Services Department) .
The Board Chairman should sign four copies of the agreement, three of which should
then be returned to the Contracts and Grants Unit for submission to State
Department of Health Services .
