Spooner, Contracts Administrator costa DATE: July 3, 1986 (out/ SUBJECT: Approval of Standard Agreement for Local Jurisdiction Special Needs and Priorities Funding (SNAP) 429-328 with the State Department of Health Services (Computerized Information Systems State Project #VI-445) SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Chairman to execute on behalf of the County, Standard Agreement/SNAP 429-328 (State Project #VI-445) with the State Department of Health Services in the amount of $40,000 for the period June 30, 1986 - June 30, 1987 for implementation of an Automated Vital Statistics System. II . FINANCIAL IMPACT: Approval of this agreement by the State will result in $40,000 of State funding for implementation of an Automated Vital Statistics System. This project is 50% State funded with 50% County in-kind matching funds required, for a total project cost of $80,000. County matching funds are included in the FY 86-87 Department Budget. III . REASONS FOR RECOMMENDATIONS/BACKGROUND: Each year the State makes AB-8 Special Needs and Priorities Funds (SNAP) available to counties . On March 18, 1986, the Board authorized submission of applications for various projects under the Distressed County Facility, Refugee Health Services, Computerized Information Systems, and Elderly Care categories for Special Needs and Priorities funding. Standard Agreement 429-328 will provide $40,000 of State Computerized Information System SNAP funds for acquisition, development and imple- mentation of an Automated Vital Statistics System (AVSS) for the Public Health Division. The AVSS was developed at the Community and Organization Research Institute at the University of California at Santa Barbara to reduce the number of times vital record information must be typed, . transcribed, mailed and key entered, thus reducing the time between an event and its inclusion in a computer-assessable data base. AVSS includes data entry capabilities for birth certificates, death certificates, and morbidity reports, greatly enhancing the value of the data for planning, tracking and medical research. Networking facilities of AVSS allow data to be electronically transmitted instantaneously to the County or State. This document has been approved by the Department's Contracts and Grants Administrator in accordance with the guidelines approved by the Board's Order of December 1, 1981 (Guidelines for contract preparation and processing, Health Services Department) . The Board Chairman should sign four copies of the agreement, three of which should then be returned to the Contracts and Grants Unit for submission to State Department of Health Services .
