Spooner, Contracts Administrator Costa DATE: 'May 1, 1986 Co "1 SUBJECT: Approval of Submission of Funding Application 4129-208-29 to the State / Department of Health Services for the Immunization Assistance Program SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and Authorize submission to the State Department of Health Services of Funding Application 4129-208-29 in the amount of $40,096 for the period July 1, 1986 through June 30, 1987 for the Immunization Assistance Program. II. FINANCIAL IMPACT: Approval of this agreement by the State will result in $40,096 of State funding for the Immunization Assistance Program. Sources of funding are as follows: $40,096 State Funding 11,681 County Personnel Costs 4,750 County In-kind (Fixed Overhead) $56,527 Total Program The '$16,431 of County costs are included in the FY 1986-87 Public Health Budget. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: On August 13, 1985 the Board approved Contract 4129-208-26 with the State Department of Health Services for continuation of the County's long-standing Immunization Assistance Program for FY 1985-86. On November 19, 1985 the Board approved Funding Application 26-208-27 and on February 11, 1986 approved Contract Amendment Agreement 4129-208-28 for purchase of a computer system and software for the program. Funding Application 4129-208-29 requests State funding for continuation of services during FY 1986-87. Contra Costa County maintains this program to make immunizations available to all persons . in need of this service, assisting all county school districts, private schools, preschools and day care centers in meeting the State-mandated immunization requirements. The program goals are to prevent the occurrence and transmission of childhood diseases preventable through immunization and to maintain immunization levels at 95% or greater for infants, children, and adolescents. The 'Clerk of the Board should return eight certified copies of the Board Order to the Contracts and Grants Unit for submission to the State Department of Health services. IV. CONSEQUENCES OF NEGATIVE ACTION: Failure to approve this application will result in loss of State funding for the Immunization Assistance Program. The County will not continue to receive free vac- cine' from the State if the contract with the State Department of Health Services is not maintained. DG:gm
