SUBJECT: Approval of Contract Amendment Agreement 422-182-3 with v�"`J/
Tri Valley Rehabilitation
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chairman to execute on behalf of the County, Contract
Amendment Agreement #22-182-3 with Tri Valley Rehabilitation effective
March 11, 1986 to amend Contract 422-182-1 (effective January 1, 1985) for the pro-
vision of additional pediatric therapy services with a $38,160 increase in the
contract payment limit.
II. FINANCIAL IMPACT:
Approval of this amendment will result in an increase in the contract payment limit
of $38,160, from $38,160 to a new total payment limit of $76,320.
Sources of funds for this contract are the State (80%) and the County (20%), and
funding is included in the Department's FY 1985-86 budget.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
The Purchasing Agent approved Contract 422-182-1 with Tri Valley Rehabilitation
effective January 1, 1985 through December 31, 1985. On November 5, 1985, the
Board approved Contract Amendment Agreement #22-182-2 to extend services through
June 20, 1986, increase the contract payment limit and number of service units
provided, and add the services of a pediatric occupational therapist . The purpose
of Contract Amendment Agreement #22-182-3 is to increase the number of service
units and the contract payment limit in order to meet the basic physical and occu-
pational therapy needs of our patients.
The contractor is used to fill in as required to relieve or augment the
Department's therapy staff. Should the Department be successful in recruiting and
hiring the requisite staff, the contractor's physical and occupational therapy
services will not be used.
This document has been approved by the Department's Contracts and Grants
Administrator in accordance with the guidelines approved by the Board's Order of
December 1, 1981 (Guidelines for contract preparation and processing, Health
Services Department) .
