Spooner, Contracts Administrator Costa DATE: May 23, 1985 4io `-"'u' "1 SUBJECT: Notification of Contractor's Request to Terminate Contract SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Acknowledge receipt of Contractor's notification of termination with regard to Contract #23-065 (Joy E. Kovalski), which was approved . by the Board on November 6, 1984, effective close of business on May 31, 1985. II. FINANCIAL IMPACT: Funding for this service was included in the Department's fiscal year budget. Source of funding for this Standard Contract was the County, and the contract payment limit is $17,500. There may be a minimal saving from the termination one month prior to the expiration of the contract. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: On November 6, 1984, the County entered into a contract with Joy E. Kovalski for the term November 7, 1984 through June 30, 1985 for consultation and technical assistance with regard to data processing operations. On May 1, 1985, the Contractor provided the Health Services Department with a thirty-day written notice of termination in accordance with the terms of Contract #23-065 (General. Conditions, 1 5. [Termination] , Subparagraph a. [Written Notice]). Therefore, the Health Services Director is recommending that the Board accept Contractor's notice of termination.
