Spooner, Contracts Administrator Costa DATE: April 25, 1985 County SUBJECT: Approval of Inter-County Agreement #24-254-6 with Marin County (Department of Health and Human Services, Community Mental Health Services) SPECIFIC REQUEST(S) OR RECOMMENDATION($) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: A. RATIFY the actions of the Health Services Director (or his designee, Jonna Stratton, Mental Health Administrator) regarding an oral'agreement with Marin County for the continued provision of subacute residential treatment. for Contra Costa County residents who are mentally disordered aid wtio need care in Marin County's Canyon Manor program, in accordance with\the Board's Order of December 18, 1984 which authorized negotiations for the below specified agreement, and B. Approve and authorize the Chairwoman to execute on behalf of the County, Inter-County Agreement #24-254-6 with Marin County (Department of Health and Human Services, Community Mental Health Services) at the rate of $48.92 per day per patient for the period January 1, 1985 - June 30, 1985 for provision of subacute residential treatment in Marin County's Canyon Manor program for resi- dents of Contra Costa County who are mentally disordered. This document inclu- des a six-month automatic extension from June 30, 1985 through December 31, 1985 with no increase in the rate per patient day. II. FINANCIAL IMPACT: This Agreement provides for payment of the Medi-Cal per diem rate ($48.92) only if or when a patient's Medi-Cal Eligibility is discontinued. As Medi-Cal Eligibility is closely monitored, it is anticipated that less than $6,000 will be expended in the fiscal year. Most patients are fully covered by Medi-Cal. Funding for this service is included in the FY 1984-85 Department Budget. Sources of funding for this Inter-County Agreement are Conservatorship funds and State Mental Health/Short-Doyle Allocation with Matching County funds. III. REASONS FOR RECOMMENDATION/BACKGROUND: The current year marks the third year that Contra Costa County and Marin County have shared the regional program in Canyon Manor which is a sub-acute residential psychiatric facility, offering 34 beds to Contra Costa County residents in need of post-hospitalization treatment. Patients are referred to Canyon Manor in lieu of the State Hospital or are placed in Canyon Manor on discharge from the State Hospital. This Agreement also provides financial security for payment of the Medi-Cal per diem rates if, for some reason, a patient is found to be ineligible for Medi-Cal. Expenditures for the per diem rate are minimal, and less costly than the cost of hospitalization. ORIG: Health Services (Contracts) CC: County Administrator ATTESTED Auditor-Controller Phil Batchelor, Clerk of the Board of Contractor Supervisors and County Administrator M362/7-83 BY DEPUTY Board Order Agreement 4424-254-6 Page 2 On October 18, 1983 the Board approved Agreement 4424-254-4 with Marin County for subacute residential treatment for Contra Costa County residents who are mentally disordered. On June 26, 1984, the Board approved Contract Extension Agreement 4424-254-5 which continued services through December 31, 1984, and on December 18, 1984 authorized the Health Services Director to negotiate an agreement with Marin County to continue services through June 30, 1985. Inter-County Agreement #24-254-6 is the result of negotiations with Marin County. This document has been approved by the Department's Contracts and Grants Administrator in accordance with the guidelines approved by the Board's Order of December 1, 1981 (Guidelines for contract preparation and processing, Health Services Department). The Board Chairwoman should sign four copies of the Inter-County Agreement, three of which should be returned to the Contracts and Grants Unit for submission to Marin County. IV. CONSEQUENCES OF NEGATIVE ACTION Canyon Manor represents Contra Costa County's most highly utilized alternative to State hospitalization. Failure to provide Marin County with a financial guarantee which would offset potentially lost Medi-Cal funds will jeopardize our access to Canyon Manor beds. Without this financial back-up, the program will be more hesi- tant in accepting our patients or will hastily transfer our patients back to the State or County Hospital. Failure to maintain this financial support will adver- sely affect our local and. State Hospital utilization.
