Spooner, Contracts Administrator Costa DATE: March 28, 1985 County SUBJECT: Approval of Novation Contract 424-297-11 with California Health Associates, Inc. for Methadone Clinic Services SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Chairwoman to execute on behalf of the County Novation Contract 424-297-11 with California Health Associates, Inc. in the amount of $483,094 for the period July 1, 1984 - June 30, 1985, and $234,000 for the auto- matic extension period from June 30, 1985 - December 31, 1985 for the continued provision of methadone maintenance clinic services. II. FINANCIAL IMPACT: The maximum program payment limit for this contract is $483,094. The sources of revenue for this contract are as follows: Fedefal Block Grant $ 85,046 Federal Medi-Cal 112,933 State Medi-Cal Matching Funds 115,491 Regular State Short-Doyle (05) 115,007 County Short-Doyle Match 12,779 County Funding for COLA 19,656 Additional County Funding 22,182 TOTAL (Payment Limit) $ 483,094 This represents a $102,127 increase over the FY 83-84 methadone program payment limit. This amount is the result of a maximized use in FY 1984-85 of the County's Federal Medi-Cal allocation for drug programs, plus the cost-of-living increase granted to contractors by the Board of Supervisors, and the shifting of approxima- tely $36,000 in Pittsburg and Richmond facility costs to the contractor that were borne by the County last year. This FY 1984-85 contract places the contractor at risk for earning the $112,933 in Federal Medi-Cal funds, so that if no federally- eligible clients are served, the County does not pay the contractor for that por- tion of the payment limit. III. REASONS FOR RECOMMENDATION/BACKGROUND: Outpatient methadone maintenance has been a treatment option for opiate addicts in Contra Costa County since 1972. The value of outpatient methadone treatment in the maintenance and treatment of opiate addicts is well documented in research in the drug treatment field. Prior to that time, the services were provided directly by County staff. This contract has proved to be cost effective, and the levels of service adequate. The most recent Board action taken on this contract was an extension of the FY 1983-84 contract through April 1985 which was authorized by the Board - on March 26, 1985. IV. CONSEQUENCES OF NEGATIVE ACTION: There are 180 clients currently in treatment in the methadone maintenance program. Methadone maintenance treatment is usually long-term (18-months or more) and requires that the client become physiologically dependent on the substance metha- done. Failure to approve this contract would result in the termination of the pre- sent contract on its current expiration date (April 30, 1985) and would require that clients on the program be involuntarily detoxified or transferred to programs in other counties. The long-term effect could be the inability to provide this cost-effective method of treatment which would have adverse consequences for the criminal justice system as methadone maintenance treatment "decriminalizes" the addict.
