SUBJECT: Approval of Novation Contract #24-336 with California Health
Associates, Inc. for heroin detoxification services
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) $ BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chairwoman to execute on behalf of the County Novation
Contract #24-336 with California Health Associates, Inc. in the amount of $30,000
for the period July 1, 1984 - June 30, 1985, and $15,000 for the automatic exten-
sion period from June 30, 1985 - December 31, 1985 for the provision of outpatient
heroin (methadone) detoxification services to County residents eligible under the
Basic Adult Care (BAC) program.
II. FINANCIAL IMPACT:
The source of funding for this contract is the Contra Costa Health Plan. The funds
have been contracted to the Alcohol/Drug Abuse/Mental Health Division for the pur-
pose of reimbursing the Division for the provision of these services. These funds
appear in the approved Annual State Short-Doyle Drug Abuse Program Budget as BAC
Revenue. The $30,000 payment limit represents no increase over last year's
contract amount.
III. REASONS FOR RECOMMENDATION/BACKGROUND:
The need for providing a medically safe detoxification from opiate addiction is
well documented. This mandatory service must continue to be provided to clients
who meet the financial and clinical eligibility requirements for the service, as '
part of the County's responsibility for providing health care services for
Medically Indigent Adults (MIA's).
This service was reimbursed under regular Medi-Cal prior to the assumption of
responsibility for MIA's by counties two years ago. During FY 1983-84, the
contract for outpatient heroin detoxification services was incorporated in the
contract with California Health Associates, Inc. (#24-297-6) for the provision of
methadone maintenance services and this year the two services are being split into
two separate contracts so that they will be easier to administer. The County has
been recently advised that the expenditure of funds from the Health Plan for BAC
detoxification services are not subject to the same cost reporting requirements as
other drug abuse funds, and should not be incorporated in a contract with funds
that do require cost reporting in the Annual State Short-Doyle Cost Report.

IV. CONSEQUENCES OF NEGATIVE ACTION:
Heroin detoxification services are a covered Basic Adult Care benefit of the Contra
Costa Health Plan. These services are provided under this contract with California
Health Associates, Inc. which is the sole provider of this service in the County.
Failure to approve this contract would result in the termination of the present
contract on its current expiration date (April 30, 1985) and would end the County's
ability to provide this service for indigents until a new provider could be
located..
