F i nucane, Health Services Director Contra Costa .DATE: March 25, 1985 County SUBJECT: Ratification of Contra Costa Health Plan Contracts SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION RECOMMENDED ACTION: Approval for membership in the Contra Costa Health Plan of the following contracts: Contract Individual Number Contracting 473 High, Jamie Ann (conversion) 474 Clark, Margaret (conversion) FINANCIAL IMPACT: Depends on use of services, size of family, Health Services Department costs, and Board approved rates. REASONS FOR RECOMMENDATIONS/BACKGROUND: The Board of Supervisors on September 10, 1980, authorized the Executive Director of the Contra Costa Health Plan to execute, on behalf of the Board, standard form individual contracts at Board established rates, subject to ratification. The contracts are executed on Contract Form #29-702 approved as to legal form by the County Counsel 's Office and the Sate Department of Corporations, subject to ratification by the Board of Supervisors. CONSEQUENCES OF NEGATIVE ACTION: All group members--including members from Medi-Cal , private groups, and Basic Adult Care--are assured the right of conversion to continue their membership in The Plan as individuals under State and Federal insurance and HMO regulations.
