In addition, the Board mandated that a plan be developed for future services which would have the support of various constituents that make up the mental health community. Our assignment included five initial tasks: 1) develop a unified planning group that was committed to developing a plan by. consensus; 2) define the population to be served; 3) develop a continuum of care; 4) develop a un ing strategy to imp ement t e continuum; an report on t e progress made to date in expanding our existing continuum with the funding a located in December. (1) Develop a Unified Planning Group The Health Services Director, Mark Finucane has convened a Mental Health Planning Task Force. In addition, two subcommittees were formed, one focusing on. children and the other on adults. Representatives of the committee have included Health Services Department administrative staff, Public Employees Local 1, A/DA/MH Division contractors, °medical staff and the Mental Health Advisory Board. `A complete list of participants is attached. (2) Definition of Populations Attached for your review are comprehensive definitions of the children and adult populations identified in this planning process. We are specifically focusing on the proportionately small , but growing., number of individuals who are severely disturbed. These individuals are the most resistant to treatment and inevitably require the most staff attention and resources. We would like the Board to be aware that this is a select population and does not reflect ,a large number of other people who have less severe problems but also require�ervices from our . alcohol, drug and mental health programs. A—41 x/81 Board of Supervisors Page 2 April 19, 1985- - I (3) Continuum of Care The A/DA/MH Division of the Health Services Department is committed to developing a treatment system that combines the medical model and the (psycho) social rehabilitation model into a unified treatment approach. We recognize that -for the populations described in this report, complete independent functioning is not always possible. We believe that the most effective treatment system is one that affords the patient (or consumer) a broad range of treatment opportunities that are tailored to his or her current level of functioning. The attached con- tinuums for children and adults reflect this approach. These continuums are extensive but, never before in Contra Costa County, have we identified the full spectrum of services required to treat severely emotionally disturbed adults and children. Each component within the continuum is progressively less restrictive and allows the individual to become increasingly responsible for his/her treat- ment plan. This treatment system is outlined in,the California Model for Mental Health Services, which was endorsed in principle by the Board of Supervisors and the County Supervisors Association of California in 1984. (4) Funding Implementation Strategy There are three tasks to be completed before we can make our final recommen- dations to the Board in June. 1) The continuums must be budgeted to determine the total cost for this treatment system. 2) The components within the continuums, not already in operation, must be ranked and a funding priority assigned. 3) A phased funding strategy must be developed that will afford your staff the opportunity to provide the citizens of Contra Costa County with a high-quality public mental health system. (5) Implementation of December 18, 1984 Allocation On March 26, 1985 a contract was approved by the Board of Supervisors with Crestwood Hospitals, Inc., to provide intensive treatment for Contra Costa County residents at 'Crestwood's San Jose Skilled Nursing Facility. The contract is to provide a subacute treatment program for up to 20 severely mentally disturbed ill adults as an alternative to Napa State Hospital . Crestwood was selected following a competitive bid process. As of April 19, 1985, twelve patients were admitted to the. San Jose facility from Napa State Hospital . It is anticipated that the program will be fully utilized by mid-May. Negotiations have also begun with Capital City Convalescent Hospital in Sacramento to patch five beds for the severely emotionally disturbed geriatric patient. This program is scheduled to begin on July 1, 1985. Board of Supervisors Page 3 April 19, 1985 On March 17, the A/DA/MH Division solicited requests for proposals for a 6-to- 12 bed children's residential program. We have received responses from La Cheim, Inc. , Victor Homes, Inc. , and Phoenix Programs, Inc. These are all highly regarded community residential programs, and a committee will soon convene to review their proposals and make a recommendation. In addition, we are in the process of adding a Mental Health Treatment Specialist to our psychiatric inpatient wards, assigned specifically to treat children and adolescents. We are also recruiting for a child psychiatrist for the inpatient wards. We believe that the work of the Mental Health Planning Task Force reflects a developing consensus among mental health constituents and outlines a direction to be followed in the years ahead.
