JORNLIN, DIRECTOR tra Social Service Department Costa DATE: March 6, 1985 County SUBJECT: APPROVAL OF LINKAGE PROPOSAL (RE: NO. 29-029) 1 SPECIFIC REQUEST(S) OR RECOMMENDATIONS) & BACKGROUND AND JUSTIFICATION I . RECOMMENDED ACTION Authorize Director, Social Service Department, to submit a proposal for $350,0001in State funds to the State Department of Aging to provide Linkage service for persons who are frail and elderly; or disabled over the age 18 and in danger of institutionalization and who are not eligible for services from mental health or vocational rehabilitation with such services to be provided during the period May 15, 1985 to June 30, 1986. This proposal is in response to a Request for Proposal issued by the State Department of Aging. II . FINANCIAL IMPACT It is proposed to request State funds of $350,000 and supplement these funds with $167,000 in Title III Older Americans Act funds and $50,000 in County Social Service Department budgeted funds. If the proposal is approved at the State and County levels, the Social Service Department will budget the $50,000 in its FY 1935-86 budget. III . REASONS FOR RECOMMENDATION The Board of Supervisors authorized a Long Term Care Pilot Project on September 27, 1983 to serve frail elderly Seniors in Contra Costa County who are "at risk" of institutionalization. The Pilot Project's Public-Private Consortium have made a commitment to continue the project through mid 1986, after which the County will continue to have responsibility for its frail elderly Seniors whether or not additional funds are available for this population. The State Department of Aging through its Request for Proposal has made $350,000 available for serving frail elderly persons; or disabled persor..s over the age 18. If the County's proposal in response to State -Department of Agirg R.F.P. is granted, it will afford Contra Costa County the opportunity to continue serving the frail elderly after the Long Term Care Pilot Project ends. BACKGROUND (AND PROPOSAL SERVICE SUMMARY) Contra Costa County began a local long term care planning and program development effort in September 1981, initiated in responseito the development of the California Long Term Care Plan in 1981 and AB 2860, the Torres-Filando Long Term Care Reform Act, in September 1982. I Throughout 1982-63 eight public and private agencies began to meet regularly to develop a local long term care plan for Contra Costa County which became the foundation for the Lonq Term Care Pilot Project. This Project currently provides case management centered, community—based long term care services to 100 frail elders. The Linkages proposal would build upon the experience gained in Long Term Care Pilot Project. The establishment of a Linkages Site in Contra Costa would not only assure the continued develop- ment of community-based long term care services within the County, it would also expand the progress made by the pilot project in providing case management services to the elderly to include this same service provision to the functionally impaired adult between the ages of 18-65. Methodology would be to establish a separate long term care unit as a Linkages Site under administration of the Social Service Department and pull together a number of the key providers of adult services within the County system. The Linkage site would work closely with the other County adult services programs for future integration. A provider council made up of current members of the Long Term Pilot Project Management Team would be formed to oversee and encourage coordination with private agencies in the County. The Linkages Site would also undertake an extensive outreach effort to alert the community to the formation of the Linkages Site as the primary point of entry for qualified adults needing long term care services. A primary goal is to have a more accessible, client-oriented system of community-based long term care services. V. CONSEQUENCES OF NEGATIVE ACTION Currently, the Social Service Department is one of eight agencies participating in the Contra Costa Long Term Care Pilot Project, providing Case Management service to 100 frail elderly "at risk" of institutionalization clients. The project' s Public-Private Consortium are committed to the project only through mid 1986. At that time the Board will be faced with determining the future of longterm care in Contra Costa County. This proposal offers the opportunity of securing $350,000 of State funding to continue the effortsjinitiated by the Long Term Care Pilot Project. REJ/SJ/do i I I
