Emergency Department and other departments of the hospital to supply A

heipital care forms and transfer forms.
Patient Admission and Discharge Duties:
Maintain and log charts in and out of department all maintain
log of patient .flow of patients currently in Emergency i,cpartment.
Stamp routine medical sheets used in Emergency Department. Complete
ordering or billing forms for services requested, goods used, or
procedures performed on patients or department use. Request medical
records from Reception and Medical Records. Time-out charts and escort
50% or transport patients admitted to the hospital . Send all forms to ward
with patient. Apply name tag if not done by the receptiunist. Facil-
itate deposition of valuables to safe and label and gather patients,
belonging and take to Lhe ward/unit.
Upon request, call appropriate medical clinic and make follow-up
appointment for patient, call family or friend to arrange for trans-
portation. Obtain discharge medications or supplies for patients as
needed. Tell family/friend in lobby the ward/area patient is being
admitted to, give directions or visiting hours. Check to see that all
nursing entries are complete, timed and signed.
Transcribing Doctors' Written Orders:
Order medicines from Pharmacy on L-3 Form, ordering food/diets
for patients. Notify and delivering X-ray requests, E.K.G. requests
to Cardio-Pulmonary and Radiology Departments, complete Laboratory
1001 test order slips and notify Laboratory. Notify Physical Therapy of
goods or services ordered. Complete appropriate forms for all
departments. Relay orders for specific treatments or medications
or therapy to charge nurse and/or nurse assigned to specific patient.
General knowledge of medical terminology.
Miscellaneous Duties and Tasks:
Order and stock routine hospital forms and supplies from Storeroom
and Busines% Office, the Warehouse and Physical Therapy. Order routine

10% from Central Supply. .Retrieve various materials and supplies from other

Take collected specimens to the Laboratory or Pathology. Order and

pJAGNOSTIC CODING OF OUTPATIENT VISITS FOR BILLING PURPOSES
Coding done in accordance with the International Classification of Diseases
9th Revision Clinical Modification (ICD-9-CH) for which special training is
90 o required. It is necessary to have an understanding of medical terminology,
anatomy and disease processes in order to correlate the diagnosie given by the
physician with the proper diagnosis found in ICD-9-CM. Some diagnostic codes
vary only slightly and the physician's diagnosis is often not e-plieit enough
may include several diagnoses. The Medical Record Technician must differentic
between the diagnoses the physician has given and the treatment recorded in ti
chart, select one diagnosis and stilt be within the payable guidelines of Medi
and Medi-Cal regulations. As a teaching facility the Medical Record Technicil

This position will perform the following:
FINANCIAL CLEARANCE
The purpose of financial clearance is to obtain accurate information
regarding the financial status of the patients in order to facilitate
collecting of revenue. The main tool of this work is a form used to
update information for the CHAS computer and the Health Plan computer;
an on-line system. If this information is incomplete or inaccurate
it will result in: loss of money to the hospital and or the patient,
delay of payments or altogether unbillable visits.
Personal information (i .e. names, aka, mother's maiden name, social
security number, •address etc.) along with billing information is re-
quired. The gathering of information used in updating begins by looking
up present and past names of all patients on a type of microfilm known
as microfiche. In this process it is necessary to view anywhere from
2 to 8 microfiche. One must have knowledge and background on the billing
requirements of Medi=Cal , Medi-Care, Health Plan and private insurances
in order to understand the information these tapes provide. The screening
process is very important in determining any prior medical treatment
at any of the County's medical facilities and eliminates duplication of
a unit record number (Medical chart). If this occurs it can result in
crucial medical information getting into one file and not the other.
This kind of mistake could result in a life :threatening situation.
Financial Clearance is performed in Emergency Room Reception, Outpatient
Specialty Clinic Reception and Dental Reception.
RECEPTION - TELEPHONES
Reception (Emergency) houses the telephone switchboard and directs all call'.
to the necessary areas. The switchboard is a source of general inforamtion
When newspapers, relatives , clergy, ambulance companies or anyone else
calls regarding the status or even physical presence of a patient the
receptionist is governed by legalities as to what can and/or cannot be
divulged.Breech of confidentiality can result in legal action against the
County.
The Code Blue phone is located in Emergency Room Reception. When this
. phone rings it is the responsibility of the receptionist to inform the
medical personnel of the whereabouts of the patient involved in a clear,
concise and accurate manner- Failure to do so could result in the pateint',
demise.
The fire alarm also is located in the Emergency Room Reception and when
it rings, the receptionist must overhead the Redstone and location, deter-
mine. whether it is a false alarm and notify the Fire Department (to stop
the fire truck) or if it is not a false alarm but is a small fire and
has been put out. In the case of the latter, the Fire Department must
still respond to be sure the fire is out. Bay Alarm must also be notified
with all the detail;'and the Hospital Engineer called to reset the alarm.
RECEPTION - HUMAN INTERACTION
Every function of Reception involves human interaction, be it with your ,
co-workers or with patients. The patients however, are the primary cause
of stress because you are dealing with physically and/or mentally ill
people. All react differently and the receptionist must be alert to any
signs of violence or acute illness. The receptionist must remain calm

Reception is the central clerical area in the hospital . We are in charge of
admissions, general information, telephone switchboard, paging, patients valuables,
and the gathering of financial information and collection of MediCal labels for
admissions both inpatient and outpatient. We are the first representative of the

D level clerk in Outpatient Specialty Clinics or in Emergency Room Reception.
Sarah Earle - ER Reception or Eleanor Lewis - Specialty Clinics.
Work is reviewed (spot checked) daily and in the case of computer rejections
returned for correction unless the error appears to be habitual . In the case
of the latter, the D level clerk or Supervising Clerk goes over to ensure complete
understanding
4. Where is your supervisor located in proximity to your desk?
D level clerk is on site. In her/his absence the Supervising Clerk is only
about 20 feet away.
5. What are the most important assignments you perform and why?
Financial Clearance because of the monetary impact.
Being a representative of the County to the patient and insuring that the patient
leaves this facility with the feeling they have received the Best medical care.
6. What is the most difficult or complex work you perform and why?
Financial Clearance is the most complex. It involves very detailed research.
The work is constantly changing with new revisions to aid in the billing process
and there is an exception to every rule. The most difficult is keeping abreast
of the current procedures.
7. What parts of your job involve the greatest responsibility and why?
My greatest responsiblity is the Code 01ue phone because this involves a life.
Second, my responsibility to the patient and to the County because one reflects
upon the other.

Yes. It is the responsibility of the receptionist to try to maintain calmness
in the event a patient becomes angry, difficult or potentially explosive. In
the event this fails the receptionist must contact either the security guard,
the "support team" or if need be the Police to ensure the safety of themselves
and other patients in the vicinity.
