I am sorry to inform you that after months of hard work, our MSSP grant proposal was
denied.
I discussed our application with State MSSP Staff who felt we had a very strong
proposal and was at. a loss to explain why we were turned down, except that "we didn't
fit the criteria" which was derived by an outside grant review team.
Many kinds of data were used in making the final determination, according to staff,
including County wide ratios fpr hospital and convalescent beds to the population,
potential caseload, and ability to manage the program.
Although our application probably had the strongest health component of any submitted
this was, evidently, not seen as important. Looking at the list of sites which were
funded, all had a strong social services orientation and an absence of an organized
system of county sponsored ambulatory=to-acute health care programs. It looks as if
MSSP will be oriented towards social service systems. I surmise that this County may
have been in a better position if the grant application came from the Social Service
Department alone, and recommend that this be done if we are_ ever in a position to apply
again.
The State MSSP Staff stated that I could tell Contra Costans that the denial was not
based on any weakness in our application, or political. considerations.
My observation is that the MSSP idea of delivering services in a coordinated approach
is the trend of the future and that our energy was not wasted. The Area Agency on
Aging commented that much of- our planning efforts will be used in their move toward
integrated system of delivering health and social services and will be adopted by
their agency.

I have received ani adopted the recommendations made by the Selection
Committee- for the designation of sites for the Multipurpose Senior
Services Projects (MSSP) . Your agency was not among the designees . A
list of the selected sites is :attached for your agency's information..
The Selection Committee considered very carefully the materials gathered
from the questionnaires submitted, the site visit reports , the geographic
distribution, populatio❑ density, available services , minority represen-
tation and special factors such as the availability of congregate housing ,
before reaching their decisions . In the process , .they concluded it would
be very difficult to test the objectives of the MSSP in .California with
only five sites . Therefore, eight sites 4rere selected to represent the
diversity or- needs and services found throughout California and to
provide significant statewide data for future planning.
de .feel confident that your agency will continue its interest in both
the MSSP effort and in serving elderly persons . Hopefully, there has
been some benefit to your agency's services program by "taking part in
the MSSP application process . As the MSSP progresses, we will keep
your agency informed.

Subj GRANT APPLICATION FOR A MULTIPURPOSE SENIOR SERVICE PROJECT
On December 12, 1978 your Board authorized the subject grant application.
At that time, we advised you that the process called for a two-stage
screening. First, the total number of applicants would be screened down
to 15. A final screening to 5 applicants will be conducted in March.
We have recently been advised that Contra Costa's application has successfully
passed the first screening so that your Board's application is now one of the
15 semi-finalists. We should know by mid-March the results of the second
screening. This memorandum should be received for information only at your
Board's meeting on February 20, 1979.

Approval of Grant Application for a
Multipurpose Senior Service Project.
The Board having received a request through the Office of the County
Administrator for approval of a grant application prepared by the Medical
Services Department for a multipurpose senior service project to provide an
intensive and coordinated set of services for senior citizens as an alternative
to placement in nursing homes; and
The Finance Committee (Supervisor E. H. Hasseltine and N. C. Fanden)
having reviewed the proposed project for the elderly and having determined it
to be cost effective in providing a comprehensive program of senior citizen
services on a capitation basis as a part of the County's Medicare HMO
(Health Maintenance Organization) ; and
The Finance Committee (Supervisors Hasseltine and Fanden) having apprised
the Board of its endorsement of this Multipurpose Senior Service Grant Proposal
on the basis that the grant will provide funding of approximately $1 .4 million
per year for administrative costs for a three-year period beginning November 1979
for approximately 800 individuals 60 years of age or over who are considered to
be "at risk" of being institutionalized;
IT IS BY THE BOARD ORDERED that the Board of Supervisors does HEREBY
APPROVE the Multipurpose Senior Service Grant Proposal and AUTHORIZES the
Director, Human Resources Agency, to submit the aforementioned grant application
to the State Health and Welfare Agency for its consideration and funding.

Attached is a summary prepared by the Medical Services Department for a
competitive grant application to the State Health and Welfare Agency.
The State is attempting to determine whether providing an intensive and
coordinated set of services to senior citizens can assist in keeping them
out of nursing homes. The pilot project will involve 800 individuals at
a time over a period of three years. The individuals must be at least
60 years old and "at risk" of being institutionalized.
In addition to comprehensive inpatient and outpatient care, the project
will provide respite care so that family members may. have some relief from
the care of an elderly family member, adult day health care, home health
services, homemaker services, health education, mental health services
and other similar services required by the senior citizen.
From the agencies who may apply, 15 semi-finalist applications will be
selected by the end of December. The final cut to the five projects which
will be funded will be made by the end of April 1979. The projects will
actually be funded beginning in November 1979 and will be for a three-year
period.
The grant will provide funding of approximately $1 .4 million per year for
administrative costs, most of which will be for staff. There will be
approximately 38 new positions funded, including twenty "case managers" .
These case managers will each have a caseload of forty. senior citizens.
It will be the responsibility of this case manager to identify a need for
specific services for his or her clients and then insure that they are
provided.
These services, rather than being provided for in the traditional fee-for-
service manner, will be provided for on a capitation basis as a part of
the County' s Medicare HMO. Each client will join the HMO and the County
will then be paid a fixed fee' each month in return for which the County
will provide or purchase from other providers all needed services.
The exact capitation premium will need to be negotiated if the County is
a successful applicant. However, it is expected that the premium will be
in the range of $750 to $850 per month. This is compared with over
$1000 per month for skilled nursing home care.
The budget which appears in the proposal is only an estimated maximum
provided as a guide by the State. Our final application will have a
detailed budget reflective of our Civil Service titles and salaries.

All positions will be "project" so that the employees can be
terminated at the conclusion .of the grant.
In order to insure that all economic levels are served, 60% of the
clients must be receiving public assistance, an additional 25% must
have incomes which are not more than 125% of the median income in the
State. The remaining 15% may have higher incomes and will be charged
for services on a sliding fee schedule.
Although the precise area of the County to be served by the project has
not been determined it probably will include Pittsburg , Antioch, West
Pittsburg and portions of Concord.
The grant application must be in Sacramento by December 22. I would
therefore request that you present this issue to the Board of Supervisors
at their meeting on December 12. The Board may wish to approve the
application then or hold it for one week. We do need a decision not
later than December 19. The action to be taken includes:
1 . Authorize the Chairman to sign the application; and
2. Authorize the submission of the application to the
State Health and Welfare Agency.

SUBJECT: HEW Special Intergovernmental Personnel Act Demonstration Program
I am pleased to announce a special HEW demonstration program under the
authority of the Intergovernmental Personnel Act (IPA). This initiative
will make available 26 carefully-selected, senior HEW personnel with skills in
human services planning, management, delivery systems, and budgeting for
special assignments in State and local government. The unique feature of this new
program is that HEW will pay up to 90% of the salary costs involved, as well as
any relocation expenses.
The program is designed to be responsive to chief elected officials in solving
problems which they believe need special attention in order to improve the
delivery of human services in their jurisdictions. As such, the approved
assignments will focus on activities which cut across health, education, and
welfare programs or on activities directed toward the goal of linking or
improving the delivery of human services. Assignments are ;possible at State
or local levels and with legislative as well as executive branch agencies.
The attached material provides additional guidelines for the program, as
well as a format for describing a proposed assignment. In order to be
considered for this program, proposals must reach us b close of business
on Friday, June 30. Proposals should be mailed directly to my new Director
of Intergovernmental and Congressional Affairs, Mr. Edward J. Avila, at the °
above address. Interim questions about the program may be directed to Mr.
Beau Carter on Ed's staff at (415) 556-2650. Allowing for the necessary
review and selection time, you should consider a starting date for your
assignment in September or October of this year.
We appreciate the fact that there are many more eligible assignments than
there are slots available. In turn, with only 26 assignments to be selected
nationwide, Region IX can probably expect no more than two to four proposals
to be approved. Nonetheless, we are making a very broad announcement of the
program to ensure that the widest possible range of eligible units of govern-
ment has an opportunity to consider submitting a proposal .
I sincerely hope that this demonstration program will provide an opportunity
for us to be of assistance to you in your efforts to improve! the delivery
of human services, and I look forward to reviewing the proposals submitted.

GUIDELINES FOR IPA ASSIGNMENT PROPOSALS
It is the policy of the Department of Health, Education, and Welfare to
assist the chief elected officials of state and local general purpose
governments to improve their capacities to plan and manage human services
programs. In order .to make these improvements, HEW recognizes the need
for experienced personnel in state and local governments to perform a variety
of tasks aimed at improving management and planning processes. The Depart-
ment also recognizes its need to understand the unique problems within state
and local governments. associated with improving these human services programs.
As part of its human services improvement goals, the Department has developed
a new program of assistance to state and local governments under the provisions
of the Intergovernmental Personnel Act (IPA) .
This new program will make available to general purpose governments 25 care-
fully selected, senior HEW personnel with skills in human services planning,
management, delivery systems, and budgeting. This program will be responsive
to chief elected officials in solving problems they believe need special
attention in order to improve the delivery of human services in their states
and local governments. The program will focus on those activities which cut
across health, education, and welfare programs and also those activities
directed to the goal of linking or improving the delivery of services.
Since NEW expects to receive many more requests for candidates than the 25
which are available, we will make the final selections competitively, based
on the best proposals from state and local governments. Proposals will be
judged on the basis of:
Technical feasibility of the proposed work assignment. ____._.

Anticipated breadth/scope of improvements in the delivery
of human services as a result of the assignment.
1 . Eligibility: Chief executives of state, county or municipal levels
of government. Legislative bodies such as state legislatures and
city councils. Elected officials of school boards and tribal
governments will also be eligible, as well as organizations whose
legitimate function is to aid elected officials improve human
services delivery.
2. Assignments: Proposals should include a detailed description of
the proposed assignment. Describe any planning, service delivery,
information systems or other improvements already being undertaken.
Describe how the HEW candidate will participate in such projects.
If the proposed assignment is a supervisory position„ a statement
of the responsibilities and authorities--including the reporting
level , budget, and personnel responsibilities--should be included.

Mr. A. G. Will, County Administrator, having recommended that the
Chairman, Board of Supervisors, be authorized to submit an application for a
Partnership Grant to Region IX, Department of Health, Education, and Welfare,
for $21,481 in Federal monies, to be matched with a County in-kind contribution
of $41,889 to assist the Human Resources Agency to identify legislative and
regulatory obstacles to maximum flexibility in the planning, delivery,
evaluation, and financing of human services for a one-year period during 1977
and 1978; and

A Proposal for a Partnership Grant for 1977-78
A. INTENT
To systematically identify and analyze State and Federal legislation and/or
regulations which:

B. BACKGROUND
Contra Costa County has had substantial experience in the planning and delivery
of comprehensive human services, including cooperative,arrangements with other
public and private agencies. This experience has included:
• Joint planning and funding of services with Model Cities Programs in
Richmond and Pittsburg

Currently under active consideration of the Board of Supervisors is the creation
of a Human Services Commission which will have substantial responsibilities for
the planning and evaluation of all human services administered by the County.
C. DESCRIPTION OF PROJECT OBJECTIVES
The proposed project is an important part of a larger effort which Contra Costa
County has undertaken to develop a coordinated human service delivery system
based on identified needs and priorities and making maximum effective use of
all resources.
The project itself addresses the major problem identified in accomplishing that
larger objective--the identification of legal and regulatory barriers.
As each functional service area is identified and its elements clarified, project
personnel will research governing laws and regulations, prepare analyses, and
determine, through consultation with appropriate State and Federal staff, the
specific steps needed to implement a coordinated human service delivery system.
Initially, the functional service areas which will be analyzed are:

• Access, information, and referral
• Continuing Care for Adults
• Employment and Manpower Training Services
• Nutrition Services
A history will be developed for each effort which will describe the issues in-
volved, the steps taken, and the response received from the State and/or Federal
agencies. These histories will provide information about the process and form
the basis of measuring success.
D. WORK OF OTHERS RELATED TO THE INTERVENTION SELECTED
Very little work has yet been done in this area. There appears to be recogni-
tion on the part of both the State and Federal agencies that it must start at
the local level. It is our intention to involve the State and Federal agencies
in the process from the beginning. To this end, we have already convened one
meeting attended by top management officials of the State Department of Health,
State Office of Benefit Payments, Governor's Office of Planning and Research,
and the Department of Health, Education, and Welfare. At that meeting there
was strong indication of support for this undertaking and agreement to cooperate
in carrying it through.
Some counties with capacity-building partnership grants will, no doubt, under-
take similar activities, and we will share information with them.
E. DESCRIPTION OF ACTIVITIES AND WORK PLAN (Refer to Gantt Chart)
The activity of the project will be continuous and sequential throughout the

project year. it will require in-depth analysis of all elements in the human
services delivery system, review and analysis of pertinent laws and regulations,
and preparation of requests for waivers from State and/or Federal agencies.
The end product will be the determination as to whether a requested waiver will
or will not be granted and the reasons why.
F. EVALUATION
The evaluation is based on the completion of a number of products by the end
of the project, along with some specific milestones which denote progress toward
that completion.

G. FEASIBILITY
Successful completion depends on both Contra Costa County and the concerned
State and Federal agencies. Top officials in those agencies have assured the
County of their desire to support its efforts.
H. POST-PARTNERSHIP CONTINUATION OF ACTIVITIES
Contra Costa County perceives planning, delivering, .and evaluating of human
services as a continuous process involving all elements of the system, the
elected decision-makers, and the community. The project activity will provide
the basis for continuing to develop a flexible delivery system tailored to the
identified needs of the County.
I. . RELATIONSHIP TO OTHER ACTIVITIES (Refer to C, above)
The proposed project is partof a larger effort which the County is undertaking
to design a comprehensive human services delivery system based on identified
needs, available resources, and established priorities. The total system will
have measurable objectives. It will seek to use resources flexibly. To accom-
plish this larger objective, the County will:
• Establish a comprehensive human services planning system (already
under consideration by the Board of Supervisors)

staff of .the Planning Department and in conjunction with United Way of
the Bay Area)

J. STATE OR LOCAL SUPPORT
As mentioned in D, above, on January 27, 1977, Contra Costa County convened a
meeting of top management officials from State and Federal agencies to discuss
its proposals for developing a human services delivery system. At that meeting,
the County was strongly urged to proceed with the effort. The County is con-
sidered by the United Way of the Bay Area as one of the major participants in
building a partnership with the voluntary agency network.
The work plan of the Office of Intergoverrurental Affairs, Region IX, HEW,
specifically refers to assisting Contra Costa County achieve its objectives.
K. POTENTIAL FOR TRANSFER
The outcomes of this project will be specific "how to" descriptions of the
process of obtaining waivers. They will have a high degree of transferability
and utility for local, State, and Federal governments. The Secretary of HEW has
given high priority to regulatory simplification. Experience has demonstrated
that this can best be initiated at the local level.

studies, analyses and research projects usually involving advance plan-
ning and scheduling. Since assignments are received in broad outline
form, incumbents are expected to exercise considerable independent
judgment in the selection of study techniques and applicable resource
material.

TYPICAL TASKS:
Conducts complex studies of the social service policies, proce-
dures, organizational structure, staffing, training, budget, equipment
and space requirements relative to the attainment of near-range and far- .
range program objectives and major program changes involving. the
delivery of services to clients; develops programs and systems to institute
new and changed approaches for the delivery of social services; prepares
.reports and recommendations to implement major program changes
directed by State and/or Federal regulations; conducts research projects
of major scope to provide the factual basis for the design of new programs-
or revision of existing programs to meet changing social and economic
patterns within the County; analyzes new or revised program directives
and regulations from the State Department of Social Welfare; prepares
recommendations for submission to the State Department of Social Welfare
regarding changes in regulations; assesses and defines local community
resources and agencies with respect to needs for new or.r.evised social
service programs; may develop plans for the coordination of services
between the social services and other public and private agencies within
the County; evaluates ongoing social service programs and submits
recommendations for revisions or modifications; develops operating pro-
cedures for the administration of demonstration projects of major scope
and provides assistance and advice during the operating phase of such
projects; conducts evaluation of projects results and plans the integra-
tion of such results into existing programs; consults wit"i other county
welfare agencies, private welfare organizations, state colleges and
universities regarding innovations in the field of social welfare; conducts
visits and meetings with all levels of departmental personnel to discuss
the effectiveness of e>dsting programs; may address public and private
groups relative to public social service programs; prepares special
studies, investigations and reports, as required.
MINIMUM QUALIFICATIONS:
License Required: Valid California Motor Vehicle Operator!s
License.
Education: Possession of a baccalaureate degree from an
accredited college or university.
E -nerier_ce: Four years of increasingly responsible paid
experience in an administrative or staff capacity in a social welfare or
social planning agency or community organization, at least three years
of which involved responsibility for planning and developing social
welfare oriented programs and operations.
Substitution: Possession of a Master's Degree in Business
Administration, Public Administration, Social Welfare, Sociology, Urban
Studies or closely related field from an accredited graduate school may
be substituted for one year of the required experience.
Thorough know'-edge of research techriqu.es used in studying
social problems, human behavior, and the characteristics of population
groins, including research design and experimental methods; knowledge .
of the fundamentals of, social casework practices as they relate to public
welfare administration; knowledge of the provisions of the California
Welfare and Institution Code and of the Social Security Act pertaining to

In accordance with your request for some additional background information
on the application we forwarded. to you on April 11 , I will attempt to
identify where this application fits into the overall goals of the Human
Resources Agency.
In my report to the Board of Supervisors last June, I identified the long-
term goal of the Agency as being "to provide maximum flexibility in the use
of available resources to meet the identified needs of the county population
in the area of human .services." In order to move toward accomplishing this
goal , the Agency Central Office staff in cooperation with the operating depart-
ments have been exploring possible program areas which would lend themselves
to block grant funding proposals. Several areas have been identified and
possible ways of administering these proposals have been developed. There
is now a need to put in a fair amount of time identifying the regulatory and
statutory constraints which prevent us from administering each of these
program proposals in .a block grant manner._ We have learned from HEW that
a limited amount of funding is available for exactly this type of effort.
This application is similar to one the Board of Supervisors approved a year
ago. That application was not funded by HEW because they were able to fund
only three projects and ours was ranked fourth out of all of the applications
submitted from Region IX.
This proposal would allow us to hire one staff person for one year ,to identify
the regulatory and statutory constraints as they relate to each of the possible
block grant funding proposals we have been developing. The concept of forming
a Human Services Commission with broad responsibility to p',an comprehensive
human services will undoubtedly speed up our efforts in the block grant
funding area. However, the submission of this application should not be seen
as being dependent on whether or not the Board creates such a Commission since
this effort is directly related to the long-term goal of the Agency and is
directly responsive to much of the material I filed with the Board last
summer.
The effort which would be undertaken by this staff person will be required
of us in order to submit any type of proposal for block grant funding. We .
simply saw this application as an opportunity to get this work done using
available federal funds rather than providing any further drain on either .
our Short-Doyle, Title XX, or county funds. If we want to do the research
which is needed to identify these constraints, we will have to use an
existing member of the county staff and the existing funding sources

available to us. If .this application is approved, it would allow us.to use the same type of staff person but take them out from under the
tight fiscal constraints we have in most of our major funding areas. .

You will recall that on May 11, 1976, the. Board of Supervisors approved
the Human Resources Agency filing an application for a Partnership Grant
with Region IX of HEW to develop a comprehensive human services delivery
system for Contra Costa County. That application was ranked fourth, of
which only three.were funded.
_The present application is a modification of the former proposal based on
guidelines issued by the Regional Office of HEW. It proposes the funding
of an effort to identify major legislative and regulatory obstacles to the
development of a flexible human services delivery system tailored to the
needs of:Contra Costa County. The total amount of Federal funds requested
is $21,481 . _ This will permit the employment of a person to work under the
direction of our Chief of Social Program Planning and Development in
carrying out the objectives of the proposal. It is part of the larger
effort which we are attempting to carry out for the development of a com-
prehensive human services system.
All of the County share is in kind and does not represent any new County
funds.
The absolute, final date for submission of the application is 'Monday,
'May 2,1977, in the office of the Regional. Director, Region IX, HEW.
Therefore, I recommend that it be placed on your Agenda for Board approval
on April 19. Final approval by the Board could be delayed until April 26.

SUbj GRANT APPLICATION TO THE DEPARTMENT OF HEALTH, EDUCATION AND WELFARE (HEW)
I am requesting authorization from the Board of Supervisors to submit a
grant application in the amount of $54,000 in Federal funds for a one-year
period to be effective July 1, 1976. The application will be made to the
Department of Health, Education and Welfare (HEW) , c/o the San Francisco
Regional Office. The purpose of, this grant is to design a comprehensive,
coordinated, and responsive system for the delivery of human services.
This system should prove to be a model for like systems, but will be
tailored to the identified needs and resources of Contra Costa County and
especially those programs administered and influenced by the Human Resources
Agency.
Both the state xnd federal offices have been vitally interested in our plan,
and Beau Carter, a liaison for the State of California Regional Director,
and Joe Maldanado, who is Director, Region IX, HEW, have assisted my staff
in the planning and preparation of the grant request.
The total budget is for approximately $100,000; $46,000 as County share.
The $54,000 project funds awarded by HEW would be used in the main for
project personnel to assist members of my staff in implementing the overall
plan. A percentage of these HRA staff persons' salaries will serve as part
of the County share along with office space and other like inkind contributions.
Personnel needs to carry out the project are as follows:
1. Director
2. Evaluator
3. Social Planner II _ project position
4. Nine months of an evaluator technician - project position
5. Clerk/Steno II - project position
6. Contract services from the Planning Department.
The last four would be 100% subvented by federal monies. The first two HRA
staff persons would be partly federally funded and part local share. They
are persons presently involved in preparation of the proposal.
The attached proposal has been discussed in detail with Gary Brown of your
staff. I am excited about extending in part the program started with Allied ,
Services. At this time, it is vital to undertake a. service needs assessment
process and plan to implement our objectives. Because of the increasingly
serious problems we are facing in relation to HRA services and the impact
on the community, we must initiate planning that emphasizes cooperation
within all County public and private service delivery agencies.
