Road and Transportation 1.1 APPROVE the Consulting Services Agreement with Fred Huang and Associates for the seismic investigation and design for the Byron Highway Bridge Rehabilitation Project, Byron area. 1.2 AWARD a contract for the Kensington Roads Improvement Assessment District Reconstruction Project, Phase IV, to the lowest responsible bidder, Hess Construction Co., Inc., who has also met the requirements of the County's Minority and Women Business Enterprise Contract Compliance Program, Kensington area. 1.3 APPROVE and AUTHORIZE the Public Works Director to execute Contract Change Order No. 6 in the approximate amount of $1,200 for the Ygnacio Valley Road Overlay Project, Concord area. 1.4 REJECT the bids received on March 16, 1995, and re-advertise for bids for the Camino Tassajara Bridge Replacement Project. Engineering Services 1.5 APPROVE an Amendment to the Blakemont Geologic Investigation Agreement between Contra Costa County and the Blakemont Property Owners Association, Kensington area. 1.6 SUB 7099: ACCEPT completion of improvements for subdivision being developed by Erselia, Limited Partnership, Alamo area. 1.7 ROAD CLOSURE: GRANT PERMISSION to completely close a portion of O'Hara Avenue in one block increments during the period March 30, 1995, through July 31, 1995, Oakley area. 1.8 ROAD CLOSURE: GRANT PERMISSION to completely close during working hours (7 a.m. through 5 p.m.) a portion of Neroly Road during the period March 30, 1995,through July 31, 1995, Oakley area. Special Districts & County Airports 1.9 APPROVE License Agreement with Pulte Home Corporation for temporary access across a portion of Dry Creek, Brentwood area. (Funding: $1,500 license fee from Pulte Home Corporation) 1.10 ACCEPT the second quarter Status & Expense Report for fiscal year 1994/1995 for the Canyon Lakes Geologic Hazard Abatement District, San Ramon area. 1.11 ACCEPT the second quarter Status & Expense Report for fiscal year 1994/1995 for the Blackhawk Geologic Hazard Abatement District, Danville area. Ordinances -None Appointments & Resignations 1.16 ACCEPT the resignation of John C. Bernard (Education Category) from the Child Care Task Force, and DIRECT the Clerk of the Board to apply the Board's policy for filling the vacancy. 1.17 APPOINT Reva Clark, Ella Gordon, Rick Gulledge, Hope Scott, Richard Spellman, Bob Sullivan, Erika Whiteway, Susan McCloskey, and Walter Wheat to the El Sobrante Municipal Advisory Council, as recommended by Supervisor Rogers. Appropriation Adjustments - 1994-1995 FY 1.18 Insurance & Risk Management (0150): AUTHORIZE $17,132 in revenues from the Workers' Compensation Trust Fund and APPROVE appropriation to add one new position in the Workers' Compensation Unit to handle the increased workload as a result of legislative changes. 1.19 General Services Fleet ORerations (0�)/Keller Surcharge/Mitigation Program (0473): APPROVE $2,420 appropriation and offsetting revenue from County lighting/landscaping service areas for purchase of an equipment trailer for Fleet Operations and APPROVE a $17,104 appropriation with offsetting revenue from the trade-in of old equipment for the purchase of equipment for the Tree Planting Maintenance Program. Legislation -None 4 (03-28-95) Personnel Actions 1.20 ADOPT resolution laying off and reappointing employees in the classification of Head Start Family Services Assistant-Project during calendar year 1995. 1.21 RETITLE the class of Chief of Security to Chief Public Service Officer and reallocate to the salary schedule; ESTABLISH the class of Assistant Chief Public Service Officer and ADD one position; ESTABLISH the class of Senior Public Service Officer and ADD four positions in the Health Services Department. (80%Federal/State, 20% County Reduction Offset) 1.22 ADD one Head Start Teacher I - Project position; one Head Start Teacher II - Project position; and one Head Start Family Services Assistant - Project position in the Community Services Department. (Federal Funds) 1.23 RECLASSIFY one Building Plan Checker II position to Senior Building Plan Checker in the Building Inspection Department. (Budgeted) 1.24 ADJUST compensation for Management and Unrepresented Nursing classifications for FY 1994-95. The proposed salary adjustment has been reviewed by the County Administrator who has determined that it will have no impact on the funding status of the County Employees' Retirement System. (Enterprise Fund) 1.25 ADOPT resolution amending Management Compensation Resolution 92/107 to apply selected provisions of the agreements with Local 1 and the Western Council of Engineers to specified management and unrepresented classes. The proposed salary adjustment has been reviewed by the County Administrator who has determined that it will have no impact on the funding status of the County Employees' Retirement System. 1.26 INCREASE the hours of one Superior Court Services Clerk III position from 20/40 to 30/40 and DECREASE the hours of one Superior Court Services Clerk Il position from 40/40 to 30/40 in the Superior Court. (No Cost Increase) 1.27 ESTABLISH the class of Fire Engineering Plans Examiner on the salary schedule and ADD two 40/40 positions; CANCEL two Fire Inspector positions in the Contra Costa County Fire Protection District. (Results in savings) Grants & Contracts APPROVE and authorize execution of agreements between the County and following agencies for receipt of funds and/or services: 1.28 APPROVE and AUTHORIZE the Chair, Board of Supervisors,to execute an agreement with the County of Alameda in the amount of $433,927 to enhance comprehensive services to Contra Costa County residents with HIV disease and their families through June 30, 1995. (No County Match) 1.29 APPROVE and AUTHORIZE the Chair, Board of Supervisors, to execute an agreement with the County of Alameda in an amount not to exceed $666,273 for continued coordination of services to Contra Costa County residents with HIV disease and their families through March 31, 1995. (No County Match) 1.30 APPROVE and AUTHORIZE the Community Services Director, or designee, to execute grant applications for the 1995 Head Start Cost of Living Adjustments 1995 Quality Improvement Funds; and 1995 One-time Funds for Improvement to the Federal Administration for Children and Families in an amount not to exceed $500,000. (In-kind Match) 5 (03-28-95) i Grants & Contracts - Continued 1.31 ACCEPT an additional award in the amount of$12,500 from the State Department of Health Services for the Black Infant Health Project through June 30, 1995. (No County Match) 1.32 ADOPT resolution supporting a grant application to the State Department of Housing and Community Development for the Federal Emergency Shelter Grant Program in the amount of $216,156 for homeless shelter operations; and AUTHORIZE the Health Services Director to submit the application along with the required certifications and assurances, to accept the grant, contract with the State and perform all responsibilities related to the contract. APPROVE and authorize execution of agreements between the County and following agencies as noted for the purchase of equipment and/or services: 1.33 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a Medical Specialist Contract with Tsu-Weng Debuene Chang, M.D. (Specialty: Urology) through February 29, 1996. (Third Party Revenues) 1.34 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Hilary Brown to increase the contract payment limit by $2,400 for additional technical assistance to the Family Maintenance Organization planning project. (Grant Funding) 1.35 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with the Center for Human Development to change the reimbursement rate to more accurately reflect the intent of the County and Contractor. (No Financial Change) 1.36 Deleted 1.37 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Therapy Connection, Inc., to increase the payment limit by $50,000 for provision of additional physical and occupational therapy services. (Enterprise Fund) 1.38 APPROVE and AUTHORIZE the Acting Social Service Director, or designee, to execute a contract with the Contra Costa County Office of Education in the amount of $27,000 for administration of the Targeted Assistance Community Work Experience Program for the period April 1, 1995,through December 31, 1995. (Federal Funds) 1.39 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Julie O. Fulford to increase the contract payment limit by $12,500 to continue audiological services through September 30, 1995. (Enterprise I Budget) 1.40 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with United Way/Opportunity West in the amount of $116,028 for alcohol and other drug primary prevention services under the "Partnership for a Drug Free Contra Costa" Project through September 30, 1995. (Federal Funds) 1.41 APPROVE and AUTHORIZE the Health Services Director, or designee,to execute a contract with Jonel Weber with a payment limit of$30,000 for provision of physical therapy services through September 30, 1995. (Enterprise I Budget) 6 (03-28-95) Grants & Contracts - Continued 1.42 APPROVE and AUTHORIZE the General Services Director to issue Amendment No. 4 authorizing $11,500 of extra work under the Consulting Services agreement dated April 20, 1994, with Fong & Chan Architects, for the Pathology Lab Air Handling Upgrade Project at Merrithew Memorial Hospital, Martinez. Leases -None Other Actions 1.43 AUTHORIZE County Counsel to determine if it is appropriate to initiate legal action to recover monies owed to the County and to initiate such action where appropriate. 1.44 RATIFY the Zoning Administrator's decision regarding Shell Oil Company's compliance with Conditions of Approval for Land Use Permit No. 2009-92. 1.45 ISSUE an order tolling the time for review of the Federal Communications Commission Form 1205's submitted by TCI Cablevision and Viacom Cablevision systems for an additional ninety (90) days. 1.46 DIRECT the Community Development Department to forward to the County Recorder a list of persons and premises for the purpose of levying liens for unpaid collection charges, pursuant to County Ordinance Code Section 418-6 on Mandatory Subscription. (Richmond Sanitary Service, Inc.) 1.47 APPROVE the change of pictures and text in the State Capitol Exhibit. 1.48 AUTHORIZE the County Administrator to release an Invitation to Submit Quotations for Revenue Collection Services. 1.49 ACKNOWLEDGE receipt'of the monthly report prepared by the Construction Manager for the Merrithew Memorial Hospital Replacement Project. 1.50 RATIFY the action taken by the County Administrator on March 21, 1995,to proclaim a local emergency within the county, including all political subdivisions therein, and to request the Governor to proclaim a State of Emergency for said jurisdictions. 1.51 WAIVE license/permit fees for the carnival co-sponsored by the Ambrose Lions Club and the Ambrose Recreation and Park District on March 30, 1995, through April 2, 1995. 1.52 APPROVE revised language of the Memorandum of Understanding Tri Valley Planning Committee Pilot Project. 1.53 APPROVE contract documents and advertise for bids to be received on April 25, 1995, at 11 a.m., for 1995-1996-1997 Exterior Fire Hazard Control (Weed abatement) for Contra Costa County (Including Bethel Island and East Diablo Fire Protection Districts), Motaga and Orinda Fire Protection Districts. 7 (03-28-95) Housing Authority M 1.54 APPROVE collection loss write-off in the amount of$8,635 for the quarter ending March 31, 1995, as recommended by the Advisory Housing Commission. 1.55 ADOPT Resolution approving submission of an application for funding under the Fiscal Year 1995 Public and Indian Housing Drug Elimination Program and AUTHORIZE the Executive Director, Housing Authority, to execute and submit to the U.S. Department of Housing and Urban Development all documents required by the application guidelines, as recommended by the Advisory Housing Commission. Redevelopment Agency 1.56 APPROVE and AUTHORIZE the Deputy Director-Redevelopment to execute a contract with Contra Costa County for partial funding of a traffic signal at the intersection of State Route 4 and Malicoat Avenue. Correspondence 1.57 LETTER from D. J. Levy, attorney representing the Mt. View and Rodeo Sanitary Districts, relative to the issuance of a Request for Proposals by the Board of Supervisors for garbage franchises when existing franchises are valid to the years 2011 and 2001 respectively. REFER TO COUNTY COUNSEL 1.58 LETTER from Elaine S. Dunlap, on behalf of Ida B. Smith, alleging "price-gouging" by the County's ambulance service, American Medical West.
