FIXING THE EMPLOYER'S CONTRIBUTION AT )

PRESCRIBED BY SECTION 22825 OF THE )
GOVERNMENT CODE )
WHEREAS: (1) The Contra Costa County Board of Supervisors is the ex-officio
governing body of the Contra Costa County Fire Protection
District and adopts this resolution in that capacity; and
WHEREAS: (2) Government Code Section 22850.3 provides that a contacting
agency may elect upon proper application to participate under
the Public Employees' (Medical and Hospital Care Act with
respect to a recognized employee organization only; and
WHEREAS: (3) Government Code Section 22754 (g) defines a county or
special district subject to the County Employees' Retirement
Law of 1937 as a contracting agency; and
WHEREAS: (4) Contra Costa County Fire Protection District is a special district
subject to the County Employees' Retirement Law of 1937 and
desires to obtain for certain of its employees and retired
employees the benefit of the Act and to accept the liabilities
and obligations of an employer under the Act and Regulations;
and
WHEREAS: (5) Contra Costa County Fire Protection District desires to obtain
for its employees who are an represented by UCOA and the
counterpart retirees and retiree survivors, the benefits of the
Act and to accept the liabilities and obligations of an employer
under the Act and Regulations; now, therefore, be it
RESOLVED: (a) That Contra Costa County Fire Protection District elects to be
subject to the provisions of the Act; and be it further

RESOLVED: (b) That the employer's contribution for each employee or
annuitant shall be the amount necessary to pay the full cost of
his/her enrollment, including the enrollment of family
members, in a health benefits plan or plans up to a maximum
of$182.85 respect to employees or annuitants enrolled for self
alone, $355.70 for self and one family member, and $475.40
for self and two or more family members plus administrative
fees and Contingency Reserve Fund assessments; and be it
further
RESOLVED: (c) That the executive body appoints and directs Leslie T. /might,
Director of Human Resources, or her designated
representative, to file with the Board of Administration of the
Public Employees' Retirement System a verified copy of this
Resolution, and to perform on behalf of the County all
functions required of them under the Act and regulations of the
Board of Administration; and be it further
RESOLVED: (d) That coverage under the Act be effective April 1 , 2002.
