Approve contract as follows:

Contractor: Mental Health Consumer Concerns, Inc.
Term: July 1, 1984 through June 30, 1985 with an automatic six-month extension from
June 30, 1985 through December 31, 1985
Payment Limit : $90,526 during the period from July 1, 1984 through June 30, 1985,
and $45,263 during said extension
Funding: County General Fund (includes $4,311 from Boad of Supervisors on
August 24, 1984 for cost-of-living adjustment)
Service: Patients' Rights Program: (1) General Advocacy and Training Services, and
(2) Certification Review Hearing Services
BACKGROUND
This contractor has been providing patients' rights advocacy and training services for
mental health patients under an automatic extension of FY 83/84 Contract 4424-238-5 which
was approved by the Boad on December 13, 1983 and Contract Amendment Agreement 4424-238-6
which was approved by the Board on August 28, 1984. Novation Contract #24-238-7 replaces
the six-month automatic extension under the prior contract.
This document has been approved by the Department 's Contracts and Grants Administrator in,
accordance with the guidelines approved by the Board's Order of December 1, 1981
(Guidelines for contract preparation and processing, Health Services Department).
