Work: DIAGNOSTIC CODING OF INPATIENT VISITS FOR BILLING AND STATISTICAL PURPOSES Coding done in accordance with the International Classification of Diseases 9th Revision Clinical Modification (ICD-9-CM) for which special training is required. It is necessary to have an understanding of medical terminology, anatomy and disease processes in order to correlate the diagnosis given by the physician with the proper diagnosis found in ICD-9-CM. Some diagnostic codes vary only slightly and the physician's diagnosis is often not explicit enough or may 90% include several diagnoses. The Medical Record Technician must differentiate between the diagnoses the physician has given and the treatment recorded in the chart, select one diagnosis and still be within the payable guidelines of Medicare and Medi-Cal regulations. The Medical Record Technician must also determine if records are complete and appropriately countersigned.
