for Contra Costa County Health Services)
Effective November 1 , 1981
WHEREAS the Health Services Department Director has submitted a
recommendation to amend the schedule of itemized service rate charges for
County Health Services adopted by Board Resolution Number 81/204, and
restate certain rates approved by Board Resolution 81/454 effective May 1 ,
1981 , and Board Resolution 81/806 effective August 1 , 1981 , and
WHEREAS the County Administrator recommends that the previous and
amended rates become effective November 1 , 1981; and
WHEREAS said recommendations have been carefully considered by the
Board;
NOW THEREFORE, IT IS BY THE BOARD RESOLVED that an amended and
restated schedule of itemized rate charges for the Health Services Depart-
ment effective November 1 , 1981 is established as follows.
