Valentine, Director `'� o'' ; �. osta Employment and Human Services DATE: October 28, 2008 U �a -. ou my SUBJECT: Zellerbach Family Foundation Grant Funding for the Shared Family Care Program SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION: APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to execute a contract amendment with Zellerbach Family Foundation, to increase the Zellerbach Family Foundation Shared Family Care grant amount by$15,000 for a total amount not to exceed $165,000 for the Shared Family Care for Young Mothers in Foster Care Project and extend the term of the grant from June 30, 2008 to June 30, 2009. (All Districts) (#29-238-1) FISCAL IMPACT: $165,000 Zellerbach Family Foundation Grant Funds (No County Match) CHILDREN'S IMPACT STATEMENT: The grant funding supports three of the five community outcomes established in the Children's Report Card: 1) "Children Ready for and Succeeding in School"; 2) "Children and Youth Healthy and Preparing for Productive Adulthood"; and, 4) "Families that'are Safe, Stable and Nurturing" by assisting young mothers in foster care to provide safe and nurturing homes for children. ABSENT: ABSTAIN: r r ATTESTED DASD TWA,CLERK OF THE BOARD OF SUPERVISORS AND COUNTY ADMINISTRATOR Contact: MICHAEL ROETZER 3-1582 EHSD CONTRACTS UNIT(EB) RHONDA SMITH,EHSD CFS BUREAU COUNTY ADMINISTRATOR AUDITOR-CONTROLLER BY: DEPUTY Page Two Zellerbach Family Foundation Grant Funding for the Shared Family Care Program BACKGROUND: The grant funds provide for the ongoing Shared Family Care for Young Mothers in Foster Care Project, serving pregnant and parenting teens, which is based on the Shared Family Care (SFC) program service design. SFC is an innovative approach to helping families achieve permanency for their children and move toward self-sufficiency. Unlike traditional child welfare services, SFC involves the placement of entire families in the homes of community members, who mentor the families and help them obtain the skills and resources they need to achieve these goals. EHSD places young women and their children in homes of community members who have been specifically trained to mentor the target population. The goal is for young mothers to obtain the skills and resources necessary to move toward self-sufficiency. The pregnant and parenting teens are given comprehensive services to meet their needs, increase their social and life skills, as well as, connected to community supports for ongoing/future relationships. EHSD identifies and trains foster parents and will identifies former foster youth who parented as teens to assist in developing foster homes and in recruiting the pregnant or parenting foster youth. The former foster youth are in a unique position to share their experiences and empowers the teens to succeed as they move forward and transition to successful adulthood.
