SUBJECT: Approval of Cancellation Agreement #26-580-1 and Contract #26-580-2 with Akindele KoFade,
M.D.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee Jeff Smith, M.D., to execute on behalf
of the County as follows:
1. Cancellation Agreement #26-580-1 with Akindele Kolade, M.D., (Specialty: Psychiatry), a self
employed individual, effective on the close of business on December 31, 2007; and
2. Contract #26-580-2 with Akindele Kolade, M.D., (Specialty: Psychiatry), a self-employed individual,
in an amount not to exceed $1,230,000, for the provision of professional psychiatric emergency
services for patients at Contra Costa Regional Medical Center and Contra Costa Health Centers, for
the period from January 1, 2008 through December 31, 2010.
FISCAL IMPACT:
This Contract is funded 100% by Enterprise I Budget. Cost to the County depends upon utilization. As
appropriate, patients and/or third party payors will be billed for services.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
On October 10, 2006, the Board of Supervisors approved Contract #26-580 with Akindele Kolade, M.D. to
provide psychiatric services for Contra Costa Regional Medical Center and Contra Costa Health Centers, for
the period from November 13, 2006 through October 31, 2009.
In consideration of Contractor's agreement to continue providing services, the Department and the
Contractor have mutually agreed to renegotiate the Contractor's legal status from a contractor subject to
withholding to an independent contractor. Therefore, in accordance with Paragraph 3. (Term) of the

Contract, the Department and Contractor have agreed to a mutual cancellation of Contract #26-580.
Approval of Cancellation Agreement#26-580-1 will accomplish this termination.
Approval of Contract #26-580-2 will allow the Contractor to continue to provide professional psychiatric
emergency services including, but not limited, crisis stabilization at Contra Costa Regional Medical Center
and Contra Costa Health Centers, through December 31, 2010.
