Lai SPECIFIC REQUEsT(S)OR RECOMMENDATION(S)&BACKGROUND JUSTIFICATION REC.ONIMENDATION(S): Approve and authorize the Health Services Director, or his designee (Jeff Smith, M.D) to execute on behalf of the County, Contract Amendment/Extension Agreement #26-433-4 with Cynthia L. Lai, a self-employed individual, effective February 28, 2008, to amend Contract #26-433-3, to increase the payment limit by 522,000, from S70,000, to a new payment limit of $92,000, and extend the term from February 28, 2008 through August 31, 2008. FISCAL IMPACT: This Contract is funded. 100% by Enterprise I Funds BACKCROUNWREASON(S) FOR RECON]NIENDATION(S): On March 7, 2007, the Board of Supervisors approved Contract #26-433-3, with Cynthia L. Lai for the provision of echocardiogram services, for the period from March 1, 2006 th.rougll February 28, 2008. Approval of Contract Amendment/Extension Agreement #26-433-4 will allow the Contractor to continue providing services through August 31, 2008.
