The project builds on the department's model of Family-to-Family services. The project has implemented a System of Care (SOC) model for four target populations who benefit from additional support. This project served to provide a more rewarding and stable situation for children and families while working to strengthen the child welfare system. The project goals are separated into two domains, participant goals and system goals and include an evaluation component. CHILDREN'S IMPACT STATEMENT This contract supports all of the community outcomes established in the Children's Report Card: 1) "Children Ready for and Succeeding in School"; 2) "Children and Youth Healthy and Preparing for Productive Adulthood"; 3) "Families that are Economically Self Sufficient"; 4) "Families that are Safe, Stable and Nurturing"; and 5) "Communities that are Safe and Provide a High Quality of Life for Children and Families," by providing evaluation and assessment of department programs to increase efficiency of service delivery. 19-045-4
