SUBJECT: Approval of Contract#27-188-3 with Gretchen D. Graves, M.D.
SPECIFIC REQUEST(S)OR RECOMMENDATIONS)&BACKGROUND JUSTIFICATION
RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Patricia Tanquary), to
execute on behalf of the County, Contract #27-188-3 with Gretchen D. Graves, M.D., a self-
employed individual, in an amount not to exceed $140,000, to provide Primary Care services to
Contra Costa Health Plan members for the period from December 1, 2008 through November 30,
2010.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan (Health Plan) member premiums. Costs
depend upon utilization.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
On December 18, 2007, the Board of Supervisors approved Contract #27-188-2 with Gretchen D.'
Graves, M.D., for the period from December 1, 2007 through November 30, 2008 to provide
professional Primary Care services to Contra Costa Health Plan members.
The Health Plan has an obligation to provide certain specialized professional health care services
for its members under the terms of their Individual and Group Health Plan membership contracts
with the County.
Approval of this Contract #27-188-3 will allow the Contractor to continue to provide Primary
Care services, through November 30, 2010.
