Dear Dr. Smith:\par
\par
\par
It is with regret I have to inform you that I will terminate my
contract with Contra Costa Regional Medical Center as of the end of
August. New major changes and commitments in my practice will not
allow me the time to continue my current involvement with the
orthopeadic department. During my long tenure working in the system, I
have developed strong relationships with a lot of special people that I
will miss greatly. The opportunity to serve the community has been
both a challenging and rewarding experience that I greatly value. I
will do my best to make the transition smooth with patient care and
proper follow up. I will be happy to explain in detail if you wish.

Sincerely, \par
