SUBJECT: Approval of Contract#27-568-2 with Lyudmila Beyzer,M. D.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND JUSTIFICATION
RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Rich Harrison) to execute on
behalf of the County, Contract #27-568-2 with Lyudmila Beyzer, M.D., a self-employed
individual, in an amount not to exceed $75,000, for the provision of professional primary care
services for Contra Costa Health Plan members, for the period from January 1, 2007 .through
December 31, 2007.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan member premiums. Costs depend upon
utilization.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
The Health Plan has an obligation to provide certain specialized professional health care services for
its members under the terms of their Individual and Group Health Plan membership contracts with
the County.
On December 7, 2004, the Board of Supervisors approved Contract #27-568-1 with Lyudmila
Beyzer, M. D., for the period from January 1, 2005 through December 31, 2006, for provision of
professional primary care services.
Approval of Contract #27-568-2 will allow the Contractor to continue providing services through
December 31, 2007.
