is disproportionately affecting children. It is the schools serving these communities and the families living in
these areas that are the focus of this project. In collaboration with the Regional Asthma Management and

currently underway in Public Health Clinics Services. The goal of the project will be to perform educational
home visits to address the home environment and to enhance asthma self-management skills by conducting in-
school asthma management education for children with asthma. This Project will also reduce school absences,
emergency room visits and hospitalizations for these children and assist in relieving some of the burdens that
families experience in dealing with this chronic disease.
Approval of Grant Award (Amendment) Agreement #28-730-2 will allow Public Health Children Services to
continue to perform Asthma related home visits to children and their families in Contra Costa County, for the
purpose of education and assistance in the self management of the disease, and mitigation of asthma triggers in
the home environment through October 31, 2007.
Three sealed and certified copies of the Board should be returned to the Contracts and Grants Unit.
