RECOMMENDATION:
Adopt Position Adjustment Resolution #20252 to ADD three (3) Medical Records Coder (VNTA-
Represented) positions and CANCEL a vacant Medical Records Technician (VNTB-
Represented) position #7267 in the Medical Records/Health Information Unit of Contra Costa
Regional Medical Center.
FISCAL IMPACT:
The Health Services Department estimates the annual cost is$180,308. The source of funding to
offset the adjustment is from County General Funding with additional offset through the cancellation
of the vacant Medical Record Technician position.
BACKGROUND:
The Health Services Department is requesting to Add three (3) Medical Records Coder positions
and Cancel a vacant Medical Records Technician position #7267 in the Medical Records/Health
Information Unit of Contra Costa Regional Medical Center. This action is part of the Department of
Health Services plan of Correction.

Historically, the Medical Records/Health Information Unit has been understaffed, which has
contributed to tremendous backlogs. Unfortunately,staff shortages and an increase in inpatient and
outpatient encounters has resulted in State violations. The Center for Medicare/Medicaid Services
(CMS) reported these violations to the U.S. Department of Health Services prompting the County to
have a Plan of Correction in place.
The duties of the Medical Records Coder include: analysis, abstracting, and coding of inpatient
discharges, outpatient surgery, Medicare Emergency department Encounters and Oncology
Medicare Patients.
CONSEQUENCE OF NEGATIVE ACTION:
Should this request not be granted, the Medical Records/Health Information Unit of Contra Costa
Regional Medical Center will be out of compliance with federal and state regulations and the
Department of Health Services Plan of Correction.
