SPECIFIC REQUESTS OR RECOMMENDATIONS &BACKGROUND JUSTIFICATION
RECOMMENDATIONN:
Approve and authorize the Health Services Director, or his designee(Jeff Smith, M.D.), to execute on behalf of
the County, Contract #26-593 with Bhupinder Singh Waraich, M.D., Inc. (dba Mission Healthcare Services
Inc.), a Corporation, in an amount not to exceed $9,150,000, to provide psychiatric staffing and leadership of
the Inpatient Psychiatric Unit and Psychiatric Emergency Services at Contra Costa Regional Medical Center
for the period from February 20, 2007 through January 31, 2010.
FISCAL IMPACT:
Funding for this Contract is 100% Enterprise I Funds included in the Health Services Departments Budget. As
appropriate, patients and/or third-party payors will be billed for services. This Contract will be cost offset
through the cancellation of contracts with individual psychiatrists and temporary staffing companies.
REASONS FOR RECOMMENDATIONSBACKGROUND:
Provision of psychiatric physician services at Contra Costa Regional Medical Center is currently provided
through an array of independent contractors and temporary staffing companies. This arrangement has proved to
be less then optimal and has resulted in a number of citations from the Centers for Medicaid and Medicare
Services.
Under Contract#26-593, the Contractor will provide staffing and leadership of the Inpatient Psychiatric Unit at
Contra Costa Regional Medical Center including,but not limited to,providing required number of psychiatrists
necessary for clinical coverage of patients twenty-four (24) hours a day, (7) seven days a week, providing a
Chief of the Department of Psychiatry to direct administrative and clinical supervision and providing
supervision all non-clinical areas related to the medical staff of the Department of Psychiatry, throu Janua

This Contractor will provide necessary psychiatric staffing coverage at the Inpatient Psychiatric Unit and
Psychiatric Emergency Services allowing County to eliminate contracts with individual psychiatrists and
locum tenens staffing companies. The current short notice coverage system is not as cost effective due to
contract psychiatrist being more expensive and not always readily available because they work elsewhere, also
locum tenens staffing companies may require a minimum number of shifts for short notice coverage which
may not always be necessary. This patchwork coverage is not only very expensive,but prevents collaboration
with other services, makes staff meetings impossible and provides these psychiatrics with little motivation to
participate in quality improvement. This Contract will provide more adequate coverage and improve the
quality, performance and compliance of the Inpatients Psychiatric Unit at Contra Costa Regional Medical
Center.
