RECO-NINIEN DATION(S): Approve and authorize the Health Services Director. or his designee (Donna Wigand) to execute on behalf of the Country. Novation Interagency- Agreement X74-193-51 with 'Mount Diablo Unified School District. a government agency-. in an amount not to exceed S620.424, to provide wraparound services for Seriously Emotionally Disturbed (SED) children and their families for the.period from Jul- 1. 2006 through June 30. 2007. This Contract includes a six-month automatic extension in an amount not to exceed S310.'12. through December 31. 'OG7. I I FISCAL IMPACT: This Agreement is funded 50°o by Federal Medi-Cal. 46% by State Earl- and Periodic. Screening. Diagnostic and Treatment Program (EPSDT), 3%by Mount Diablo Unified School District and 1% by Mental Health Realignment. CHILDREN'S IMPACT STATENIE\T: i This program supports the following Board of Supervisors' community outcomes: "Children Read- For and Succeeding in School": "Families that are Safe. On December 6. 2005. the Board of Supervisors approved Interagency Agreement =74-193-3 (as amended by Contract Amendment Agreement =74-193-4) with Mount Diablo Unified School District, for the period from Julv 1. 2005 through June 30. 2006, (which included a six-month automatic extension through December 31. 2006) for the provision of wraparound services for Seriously Emotionall} Disturbed (SED) children and their families. Approval of-Novation Interagency Agreement=74-193—? replaces the automatic extension under the prior agreement. and allows Agency-to continue providing services through June -30.2007. I I I i i I
