FISCAL IMPACT: No fiscal impact is associated with this request. However, if legislation is successful and the Contra Costa Health Plan receives !approval to implement a long term care integration pilot program, Medi-Cal and Medicare funds would become available to Contra Costa. BACKGROUND: Article 9, Section 14499.90 of AB 2979 (Richman) would, if adopted, authorize implementation of Access Plus with Community Choices plans in two counties. For over a decade, Contra Costa County has been seeking the opportunity to establish an in- tegrated system of long term care for seniors and adults with disabilities. This proposed legis- lation would make it possible for us to implement a coordinated system of care for Medi-Cal eligible seniors and adults with disabilities. Since the adoption of AB 1040 long term care legislation in 1995, our county's Health Services and Employment and Human Services Department staff, along with community stakeholders, have been working towards the establishment of a seamless system of care. This proposed system of care would promote cons Iumer choice and independence allowing vulnerable adults and seniors to live in their own homes for as long as possible, avoiding unnecessary hospitali- zation and institutional care. As proposed, Access Plus with Community Choices would include Medi-Cal managed care and Medicare funding in the pilot project. The pilot project would make it possible to offer 30,000 low income seniors and adults with disabilities who reside in Contra Costa County a choice of home land community based service options coordinated with their health care and would maximize their ability to live as independently as pos- sible in their own homes. r
